objective: To try to find a new way to diagnosis female bladder neck obstruction, to decide the right length and right position in the transurethral bladder neck incision, to evaluate the effect after the operation and if it has evidence for re-operation.Patient and Methods: 5 female patient were studied. the average age is 66 years old. The preopration examine involved Urodynamic checking and bladder endoscope. the endoscope show the bladder neck upper and bladder wall trabecula hyperplasia. With the urethra pressure profile, we cut the bladder neck in 4 and 8 clock point, the length was the front 1/3 part of the urethra and the depth is 0.6cm and no deeper than 0.8cm.Result: All patient can micturate. the averate maximum flow rate is 18ml/s. the average residual urine volume is 15ml. .No case shows urinary incontinence, urinary fistula, haemorrhage and second operation. And the postoperation urethra pressure profilometry show the front pressure is down and become low and flat.Conclusion: The Urethra pressure profilometry have some value in diagnosis and treatment of female bladder neck obstruction,especially in determine the right incision length in operation and evulate the postoperation effect.
