Study On The Expression Of Plasma CagA And VacA, Gastric Epithelial Cell Proliferation And Apoptosis In Chronic Superficial Gastritis With Spleen-Stomach Damp-Heat

Objective: To investigate the expression of Cytotoxin associated gene A(CagA) and Vacuolating cytotoxin A (VacA) ,gastric epithelial cell proliferation and apoptosis of chronic superficial gastritis with helicobacter pylori(Hp) infection and Spleen - Stomach Damp-Heat. Subjects and methods:58 Hp positive chronic superficial gastritis (CSG) patients were selected as subjects. According to the differentiation standards of diarrhea of TCM, 32 cases belonged to spleen-stomach danp-heat(SSDH,17male,15 female,with an average age of 43.61 10.77, 19-64 years old),26 cases were spleen-stomach qi vacuity(SSQV,12 male, 14 female,with an averge age of 44.18 11.02, 22-65 years old), 12 heathy persons as control group(6 male,6 female, with an averge age of 42.53 10.03, 20-63 years old).The immunoblot assay was used to detect serum antibodies to CagA and VacA obtained from the patiens infected by H.pylori.Terminal deoxynucieotidyltransferase mediated dUTP nick end labelling(TUNEL) technique and proliferative cell nuclear antigen(PCNA) immunohistochemical staining were applied for investigation.The severity of inflammation was assessed with sections under microscope. Result:1. The positive rate of anti-CagA IgG and anti-VacA IgG were much higher in SSDH than in SSQV(p<0.01 p<0.05).2. PCNA labelling index(LI) of SSDF was significantly higher than that of SSQV(p<0.01),the apoptosis index (AI) in the two groups was of no significantly different(p>0.05).The LI and AI in the two groups were significantly higher than that of normal controls group(p<0.01).3. The severity of gastritis in SSDH was higher than that in SSQV(p<0.05),the degree of inflammation in Hp positive patients was significantly higher than that of Hp negative patients (p<0.01).4. There was no correlation between LI or AI and the severity of gastritis(p>0.05).5. The LI and AI had a positive correlation in SSQV group (r=0.546, p<0.01, n=26)and normal controls group(r=0.812, p<0.01,n=12), The LI and AI also had a positive correlation in SSDH group ( r=0.382, p <0.05, n=32 ) ;AI/LI in SSDH group was significantiy lower than SSQV group and normal controls group(pO.Ol), AI/LI was no significantly different between SSQV group and normal controls group(p>0.05).Conclusion:1.SSDH has a significant microbiol situation.CagA may serve as areference index on TCM differentiation syndromes.2. Inflammation is more severe in SSDH.3. The imbalance of apoptosis and cell proliferation may be a tigger factor for cell hyperproliferation,resulting carcinogenesis.
