PrefaceAcute renal failure ( ARF) is one of main complications in case of post operation in obstructive jaundice ( OJ). Many investigation display that ARF is the result of several agent interaction. In recent years , people detect that there are close relationships between blood vessel active material and kidney lesion of OJ . Nitrogen monoxide (NO) and endothelins(ET) are antagonists of blood vessel active material, and they are important at the occurrence and development of kidney lesion in obstructive jaundice.At present, there are many research reports about drug treatment of kidney lesion in OJ. But these methods are often limited to the disposal of simple agent and the results in clinical application are not obviously. It has been confirmed that recombinant human growth hormone (rhGH) , by the way of protecting gut mucosa barrier, can decrease the level of endotoxin and tumor necrosis factor a. (TNF-a) . At the same time, rhGh and its local effector, insulin - like growth factor - 1 (IGF - 1) , have important promoting action in the functionof kidney, growth of kidney , and regeneration of injuried tubular epithelial cells. But there are no reports whether kidney lesion can be released through affecting the concentration of NO and ET - 1 in plasma. Therefore , it is significant to pay more attention to this aspect.In this study, we would research the effect of rhGH on ET - 1, NO and kidney function through build - up of experimental OJ models with rats and probe into the clinical application.Materials and MethodsExperiment animals; 45 adult male wistar rats with the body weight of 300 - 350 grams. Methods;1. Dividing the 45 rats into three groups in random and average A: sham operation group( SO) , 15 ; B:obstructive jaundice group( OJ) 15 ;C: OJ +rhGH group, 15.2. The rats were fixed on the operation table, and incised with knife to open the abdominal cavity. Then we exposed the hepatoduo-denal ligament to find the common bile duct. The rats in B group and C group suffered from bile duct ligation, while the control group was not. After doing all these, we closed the abdominal cavity with thread.3. Twelve hours later , the rats were allowed to eat, drink and move freely.4. Two weeks later , OJ + rhGH group rats were injected with rhGH subcutaneously on the left lower leg once a day. The dose was 0. 75iu/100g, and the other two groups were injected with saline just with the same dose and on the same part.5. Three weeks later , the animals were killed with livers and blood gathered. .6. The plasma was taken out of blood to detect the bilirubin,ET -1,NO,Grand BUN.7. Kidneys were composed into tissue section with HE stain which was examined under light microscope.ResultThe manifestation of jaundice in group OJ and OJ + rhGH occurred after 1 week or so , with significantly higher level of TBIL and DBIL than that in group SO . This difference showed the successful build - up of OJ model. The level of NO in group OJ(67. 61 ±9. 50μmol/L) was significantly higher than that in group SO(25. 85 ±5. 55μmol/L,P < 0. 01 ) and that in group OJ + rhGH (45. 80 ± 5. 56μmol/L,P<0.01).The level of ET - 1 in group OJ(43.70±13. 72 pg/ml) was much lower than that in group S0( 114. 87 ± 12. 29pg/ ml,P<0.01) and that in group OJ + rhGH(94. 27 ±10.25 pg/ml,P<0. 01). The level of Cr and BUN in group OJ(43.13 ±3. 86 mmol/ LJ2.27 ± 1. 50 mmol/L) was higher than that in group S0(32. 33 ± 3. 29 mmol/LJ. 10 ±0. 66 mmol/L,P <0. 01 ) and that in group OJ+ rhGH(35. 33 ±3.47 mmol/L,8. 41 ±0.77 mmol/L, P <0. 01). Under light microscope, normal structure of renal corpuscles and renal tubules can be seen in the tissue section with HE stain in group SO. In group OJ , it can been seen that renal cells had gotten degenerated and inflammatory cells aggregated. Bile pigments granule can been seen in renal tubular cytoplasm and cavity.
