Obstructive jaundice is a common disease in general surgery. Surgical operation is a traditional and effective way to solve obstructive jaundice. But immune disorder, endotoxemia and multi -organ injury in perioperational period make the mortality high. This problem is difficult to solve in bile duct surgery for many years. Obstructive jaundice may cause a series of complications, such as infection, endotoxemia , liver function injury, liver - kidney syndrome and nutrition disorder. Infection is a usual reason of death for obstructive jaundice patients. Many experts believe that infection complication is related closely to immune disorder. Interleukin - 2 and soluble interleukin - 2 receptor are important data to reflect bodys immune function. So increasing immune function is a very important method to decrease infection complication for obstructive jaundice patients and decline the post - operative mortality. As we all know, traditional method to increase immune function is using albumin, thymopentin, etc. But the effect is not definite. Recently, the role of recombinant human growth hormone to increase immune function has caught people's attention.But the study about the prevention and treatment of somatotrophin to obstructive jaundice has not been reported in China. The purpose of our study is to discuss the protective function and mechanism of somatotrophin to experimental obstructive jaundice.ObjectivesThrough setting up obstructive jaundice mouse model , interfering with somatotrophin and designing control group and positive group randomly we detect blood interleukin - 2 and soluble interleukin - 2 receptor. According to the experimental result we discuss the protective function of somatotrophin to immune system in obstructive jaundice, and thus offer a potential therapeutic strategy to obstructive jaundice diseases.Materials and MethodsExperiment animals; 45 adult male wistar rats, body weight 300 -350 grams.Methods :1 . Dividing the 45 rats into three groups in random and average. A: sham operation group (SO), 15; B; obstructive jaundice group (OJ) 15;C: OJ+rhGH group, 15.2. The rats were fixed on the operation table, and incised with knife to open the abdominal cavity. Then we exposed the hepatoduo-denal ligament to find the common bile duct. The rats in B group and C group suffered from bile duct ligation, while the control group not. After doing all these , we closed the abdominal cavity with thread.3. Twelve hours later the rats were allowed to eat and drink freely and moved freely.4. Three weeks later OJ + rhGH group rats were injected with rh-GH subcutaneously in the posterior legs once a day. The dose was 0.75iu/100g, and the other two groups were injected with normal saline just the same dose.5. Four weeks later the animals were killed and blood was gathered.6. The serum was taken out of blood to detect the bilirubin, IL -2 and sIL - 2R in the three groups.ResultsSome obstructive jaundice mice became yellow in tail and ear in fourth day. Majority of them became yellow totally in seventh day and the urine was deeply yellow. Control group didnt have above changes. Our experiment ended up after four weeks. When we opened the abdominal cavity again, we found the common bile ducts were enormously enlarged just above the ligated level, and the wall was very thin. The result of blood bilirubin value agreeed with obstructive jaundice. IL-2: 50(155.595 ±46.328pg/ml) , OJ(34.151 ±7.583pg/ml) , OJ+rhGH(69.332±16.208pg/ml). s!L-2R: 80(6.972 ±0.674 pg/ml), OJ(12.343±1.327pg/ml), OJ +rhGH(9.014 ±0.661pg/ ml).DiscussionThere are many impairments for obstructive jaundice to immunefunction, especially to cell - medicated immune function. The high complication incidence rate in obstructive jaundice patients is due to the impairment of immune function and the decrease of bodyfe protective ability.
