Objectives: In recent years, more and more researchers have thought that inflammation is an essential feature during the occurrence and development of coronary artery disease. So the stimuli and the trigger mechanism become the field that many researchers focus on. As the receptor of many microbial products, CD14 will stimulate monocyte/ macrophage,vascular endothelial cells and smooth muscle cells etc. to secrete cytokines,growth factors and upregulate adhesion molecules after CD14 combines with its ligands. So CD14 may play an important role during the occurrence and development of coronary artery disease. But most of the studies about CD14 only regarded CD14 as the marker of activated monocytes except for the Koenig's study. Koenig etc. have discuss the relationship between sCD14 and sensitive systemic markers of inflammation in their study. But they only enrolled patients with stable angina pectoris and controls. To my knowledge, no data has been published about the relationship between sCD14 and systemic markers of inflammation in patients with acute coronary syndrome. We discussed the status of sCD14 during the occurrence anddevelopment of coronary artery disease by analyzing the plasma level of sCD14 in patients with stable angina pectoris and with acute coronary syndrome and controls, the relationship between sCD14 and CRP which is systemic marker of inflammation and the relationship between sCD14 and vWF which is the marker of endothelial cells' function.Methods: The plasma levels of sCD14,CRP,vWF in 54 male patients with coronary artery disease and 11 male controls were measured by ELISA. 54 patients were divided into three groups: stable angina pectoris group( SA, 16 patients); unstable angina pectoris group( UA, 19 patients); acute myocardial infarction group( AMI, 19 patients). We analyzed the difference of the plasma levels of sCD14,CRP,vWF in different groups, the relationship between sCD14 and CRP, the relationship between sCD14 and vWF by SPSS 10.0 for Windows.Results: 1.The plasma level of sCD14 in AMI was (12.17±8.03)μg/ml, that in UA was (9.18±7.49)μg/ml, there was no significant difference between them(P>0.05). But there were significant increase when the plasma level of sCD14 in AMI and UA compared with that in SA(3.86±2.58)μg/ml and controls(2.10±0.93)μg/ml, respectively (AMI versus SA, P<0.01; AMI versus controls, P<0.001; UA versus SA, P<0.05; UA versus controls, P<0.01). But there was no significant difference betweenthe plasma level of sCD14 in SA and that in controls(P>0.05). 2. The plasma level of CRP in AMI was (10.38±6.04) mg/L. It was higher than that in UA(9.18±7.49) mg/L, but did not reach statistical significant(P>0.05). There were significant increase when they compared with the plasma level of CRP in SA(2.69±2.24) mg/L and controls(1.61±1.48) mg/L, respectively (AMI versus SA, P<0.001; AMI versus controls, P<0.001; UA versus SA, P<0.05; UA versus controls, P<0.01). But there was no significant difference between the plasma level of CRP in SA and that in controls(P>0.05). 3. The plasma level of vWF in AMI was (174.35±62.36)%, that in UA was (152.56±35.93)%, there was no significant difference between them(P>0.05). But there were significant increase when the plasma level of vWF in AMI and UA compared with SA(97.58±41.72)% and controls(96.00±25.84)%, respectively (AMI versus SA, P<0.001; AMI versus controls, P<0.001; UA versus SA, P<0.01; UA versus controls, P<0.01). But there was no significant difference between the plasma level of vWF in SA and that in controls(P>0.05). 4. The plasma level of sCD14 showed correlation with the plasma level of vWF( r=0.64, P<0.001). The correlation existed after controlling for age,group,counts of narrow coronary artery,the plasma level of CRP(r=0.49, P<0.001). After linear regression, we got the linear equation: Y=-3.62+0.082X. 5. The plasma level ofsCD14 showed correlation with the plasma level of CRP( r=0.57, P<0.001).
