| This study try to investigate the relationship of femur intramedullary operation and the occurrence of fat embolism, to find out the effects of improving surgery technique and instrument and selecting a new pattern material on reduce the intra- and/or post-operation incidence of fat embolism. As the body response for the trauma, the adipose tissues as blood serum behavior in the wounded tissue were suffered mobilization. At the same time, the ectogenesis fat tissue entranced blood circulation. As the result of hemodynamics alteration, there were blood platelet and erythrocyte and leucocytes and lipid granules collecting on the surface of these lipid droplets. The volumes of these lipid droplets were enlargement. These lipid droplets were changed to fat embolus by the promotion of coagulation and fibrin deposition intra-blood vessel. The fat embolus, the mechanical emphraxis matter to blood vessel and with chemo-toxicity to tissue, lead to clinical syndrome that major manifestation was dyspnoea. The fatembolism influenced cardiopulmonary function severely. Femur intramedullary operation was the important conformation of the modern orthopedics' technique. It involved four components those were expanding medullary cavity with drills and femur preparation with metal rasps and bone cement filling and prosthesis or intramedullary pin insertion. Femur intramedullary operation could release mass lipid droplet into medullary cavity and make blood sinusoid of bone marrow disruption and increase medullary cavity pressure. With some other factors, the fat embolic events were common during operating in femoral medullary cavity. In order to reduce the intra- and/or post-operation fat embolic incidence, it was important to be done that renewing operation technique and improving tools and re-designing prosthesis and intramedullary pin and finding out the new type of filling material. |
