Backgroud: A series of changes in the gut mucosa after serious burn . cause microorganisms and endotoxin from the gastrointestinal tract through the injured intestinal barrier to liver or other organs,which leads to damage organs and infeaction. Attention of ecoimmunutrition by which prevent infection and control intestinal inflammation is paid by expests in medical field. However, there is no report about synbiotics used as ecoimmunutrition in serious burn patients at home and abroad. So we designed the following experiment.Objective : To explore the effect of early enteral nutrition with synbiotics on the plasma endotoxin levels and infection complications in serious burn patients .Methods: Forty cases of patients with serious burns were randomly divided into treatment and control groups. Twenty cases in treatment group were given by synbiotics (plantarum , paracacei sp paracasei,lactococcus and pediococcus plus 2 gram of each of thefollowing four fibers:Cellulose ,Starch,Pectin and Inulin.) ; twenty cases in control group were given by only above same four fibers with early enteral nutrition. Plasma endotoxin level were examined on the days 1,3,7,10,14,21 after serious burnsing. And also the changes of the infection complication morbility and mortality in the two groups were compased.Results: On the 10th days ,the plasma endotoxin level of the treatment group is obviously lower than that in the control group (p<0.05 ) . Although the plasma endotoxin levels of the treatment were also lower than that in the control group on other days , no statisticsly differences were found. The incidence of Sepsis was 15% in the treatment group compared to 25% in the control group. And the incidence of total infectious complications ( including lung infection ,urinary tract infections and blood infection) was 40% in treatment compared to 55% in control group. However thers are no statistical significance between the two groups. 10% patient died in the treatment group and 5% in the control group.Conclusion: Early enteral nutrition with lactobacillus synbiotics used in serious burn patient might reduce the plasma endotoxin level and has a potential effect in control the infection.
