IntroductionFibrosis and scar tissue formation peridual after laminectomy can cause adhesion of dural and the region around nerve root, the dural compressed and the glide of nerve root restricted. These can be the reasons of sciatica and low back pain and failure of operation. It is difficult to prevent this complication in orthopedics surgery. In this test, collagenase was used to prevent at peridural adhesion after laminectomy and obtained the first step results.Material1. Animal: 90 Rabbits, weigh range 2. 0 -2. 5Kg.2. Drugs: 2% Sodium phenobarbital, 0. 5% procaine, 0. 9% Natrium chloride, 2%Sodium hyaluronate, collagenase.3. Instrument: light microscope, slice machine.Method90 rabbets are divided into 3 groups at random. 2% sodium phenobarbital was injected into peritoneal cavity (30mg/kg) as basic anesthesia. Local anesthesia was performed using 0. 5% procaine. Exci-sing lumber vertebral plate with rongeur under sterility. The dural is not be injured. We choose 3 different treatment. Group A fill with 0. 9% Natrium chloride. Group B fill with Sodium hyaluronate and at Group C Collagenase was used. After waken up, the rabbits were feed alone under the same condition and examed in 2,4,8,12 weeks random.Naked eye observation:Irrelevant surgeons were invited to dissect animals in each group under operational microscope. Evaluation was carried out according to modified Rydell and Balazss standard.The light microscopy observation:spinal column including muscles were completely take out from every animal fixed using 10% formaldehyde for 48h. 5% nitric acid was used to take off the calcium for 2 weeks. The vertebral which plate was lack was divided into 3 segment equally, dehydrating and embeding with wax. In each segment, there were 2 slices, observed under light microscope after HE dyeing. Scored by a pathologist according to modified Nussbaum criterion basting on dural adhesion are-a, peridural collagen density and the degree of scar tissue or the filling extend to lateral and posterior of vertebra canal.Statistical management: score of naked eye and histology were a-nalysised by SPSS10.0 and Q test.ResultsNaked eye observation:Group A: The dural is adhesive, but can be separated at 2 weeks. At 4 weeks, the scar tissue did not cut down and it is difficultto separate the dural from conglutination. The scar tissue is hardened and adhered completely. It can not be separated bluntly.Group B; At 2 week, dural conglutinated granulation tissue at back. 4 weeks later, Adhesion increased and concentrated in back region. The adhesion is more gentry than that of Group A. The thick adhesion was formed at 8,12 week at back of dural. But, the difference is not significant compared to that of Group A (P >0.05 ).Group C: The space outside dural is obvious, no adhesion occurred. The adhesion of Group C is mild compare to that of Group A in each time point ( P <0. 05 ) .Light microscope observation:Group A: The outside dural space was full with granulation tissue fibroblast and osteoblast hyerplasia actively. Adhesion is widely. At 4 week, most of the fibroblast transit to fiber cell. Vascularity is decrease. At 8 week, most of interstitial tissue fibrosis collagen is thick. Adhesion is tightness. 12weeks later, the vertebral plate regeneration completed. Vertebral deformity is obviously Dural attached to new born plate.Group B: 2weeks, granulation tissue was filled at back of dural. Adhesion was formed. Fibroblast hyperplasia appeared, but it is not as active as Group A. Medicine composition was seen at lateral vertebral canal at nerve root region. Neutral white cell reaction is obviously. At 4 weeks, no medicine composition be seen, inflammation cell reaction alleviated. The adhesion of dural at back is more than that of 2 weeks. But these is not obvious adhesion or only slight adhesion at lateral vertebral canal, collagen is thick.
