Study On Relationship Among TCM Syndrome, Tongue Picture & Apoptosis Of Exfoliated Cells In Type 2 Diabetes Mellitus

Obejective: To study the relationship among TCM (Traditional Chinese Medicine) syndromes, tongue and apoptosis of exfoliated cells on T2DM (Type 2 Diabetes Mellitus) . In order to offer objective index to TCM syndromes, tongue diagnose and therapy T2DM.Methods: T2DM were divided into 5 groups according to TCM syndrome, every group has 12 cases as research subjects, 60 cases in all. Observe tongue pictures, print tongue exfoliated cells by Ba shi staining, account MV (Mature Value) of exfoliated cell of fur; Apoptosis was evaluated by means of terminal deoxynucleotidyl transferase mediated -d-UPT nick end labeling (TUNED , and immunohistochemisty were used to detect the protein of Bcl-2. 30 healthy persons as comparitive group. Study relations among TCM syndromes, tongue pictures, FPG (blood glucose on an empty stomach) , 2hPG (After 2 hours Blood Glucose), HbA 1 c (Hemoglobin Glycosylated) to tongue pictures, MV, AI, Bcl-2.Results: (1) Most of the ligules are red dark tongues, red tongues ; the furs are yellow sticky or white sticky or thin yelloew. (2) MV are conspicuous difference between T2DM and healthy persons(p<0.01). MV are the highest in interior caused by yin deficiency syndrome, the lowest in both yin and yang deficiency syndrome, MV are conspicuous difference on differential syndromes(p<0.01 or p<0.05); MV are the highest in light red tongues, the lowest in red dark tongues, MV are conspicuous difference on differential tongues (p<0.05); MV are the highest in thin tongues furs, the lowest in little furs, MV are conspicuous difference on differential furs (p<0.01 or p<0.05). (3) AI of tongues are conspicuous difference between T2DM and healthy persons(p<0.01 or p<0.05). They are the highest in interior caused by yin deficiency syndrome, the lowest in both yin and yang defieciency syndrome. They are conspicuousdifference on others syndroms(or p<0.05); AI are the highest in red tongues, the lowest in light red tongues, They are conspicuous difference on others tongues (p<0.01 or p<0.05); AI are the highest in thin yellow tongues furs, the lowest in little furs, They are conspicuous difference on others furs (p<0.01 or p<0.05). (4) Ratio of Bcl-2 postive expreesion of tongues are conspicuous difference between T2DM and healthy persons(p<0.01). They are the highest in both yin and yang defieciency syndrome, the lowest in both qi and yin deficiency syndrome, the energy-stagnation and blood stasis syndrome is similar to wetness-evil and plegm besieged syndromes, they are conspicuous difference than others syndromes(p<0.01 or p<0.05). Bcl-2 are the highest in purplish red tongues, the lowest in light red tongues, it is conspicuous difference on others tongues (p<0.01); Bcl-2 are the highest in thin yellow moist tongues furs, the lowest in thin yellow furs, conspicuous difference on differential furs (p<0.01 or p<0 05). TCM syndroms and tongue pictures are not relate to the classfication of Bcl-2 postive expreesion. (5) MV are not conspicuous difference on both differengtial FPG level and differengtial 2hPG level (p>0.05). MV of HbAlc<7% are conspicuous higher than HbAlc>7%, MV is oppositly relate to HbAlc (r=-0.589) . AI are conspicuous difference between FPG^8.3mol/L and < 8.3mmol/L (p<0,05). Ratio of Bcl-2 postive expreesion of tongues are the highest on FPG 7.0-8.3mol/L and the lowest on ^83mmol/L(p<0.05). AI of tongues of 2hPG ll.l~13.9mmol/L are the highest, conspicuous difference on the others(p<0.05), Ratio of Bcl-2 postive expreesion of tongues are not conspicuous difeernce on diferential 2hPG (p>0.05); Both AI and Ratio of Bcl-2 postive expreesion are not conspicuously relate to HbAlc(p>0.1). FPG, 2hPG, HbAlc are not conspicuously relate to the classfication of Bcl-2 postive expreesion. (6) With increasing of ratio of Bcl-2 postive expreesion of tongues, AI is increasing and then decreasing.Conclusion: (1) Red or crimson tongue, yellow or white with moist, thin yellow furs are majority tongue pictures of T2DM, it correspond the tongue pictures of qi deficiency, yin-deficiency are eesen...
