Purpose To analyse and compare the helical CT with DSA findings of hepatic arteriovenous fistula (AVF), and to evaluate the value and the limitation of helical CT Scanning in the diagnosis of AVF. Material and Methods 89 Patients With clinical suspicion of Liver lesions received DSA of the liver. Of 89 cases, 55 received dynamic double-phase helical CT scanning of the liver,and 34 received the scanning except the hepatic arterial phase(HAP). The criteria for diagnosing AVF in the DSA were as follows: early opacification of the portal vein, hepatic vein, or inperior vena cava during the HAP. The criteria in the CT were as follows: (Dearly opacification of the portal vein during the HAP;(2)the area with transient hepatic parenchymal enhancement during the HAP was isoattenuation or slight hyperattenu- ation during the portal venous phase(PVP) and precontrast CT scan- ing;(3) early opacification of hepatic vein during the HAP Result 19AVF were detected by DSA in the 89 cases (21. 35%). Of 34 cases without the scanning during the HAP, only 1 of 6 AVFs was detected by CT (16.67%) ;whereas, of 55 with dynamic double-phase helical CT scanning , 10 of 13 AVFs were detected (76.92%) (p<0.05) .Conclusion (Dearly opacification of the portal vein, hepatic vein, or inperior vena cava during the HAP is a reliable CT features with AVF, and transient hepatic parenchymal enhancement during the HAP is another feature of AVF; (2)there is a good relationship between dynamic double-phase helical CT scanning and DSA about showing AVF;double-phase helical CT scanning is greatly valuable in the diagnosis of AVF occuring in the main portal vein, main hepatic vein and their large branches;whereas it is limited when AVF occurs in the small branches of the portal vein or hepatic vein.(3)CT is an important supplement for the patients when there are the complicated vessles of the liver;(4) double-phase helical CT scanning is necessary for diagnos- ing AVF;(5) the delay time of the HAP plays a major role.
