OBJECTIVE:To explore the value of VCTDSA in the diagnosis of intracranial vascular malformation.METHODS:50 patients in intracranial vascular malformations with the VCTDSA and 3D-DSA data were analyzed retrospectively. Observation targets included position, size, type, feeding arteries and draining veins,intracranial steal phenomenon and merging lesions. The diagnostic sensitivity and specificity of VCTDSA were calculated with the gold standard of 3D-DSA, and possible factors that affect diagnostic were analyzed.RESULTS:50 patients were single lesion, VCTDSA detected in 36 cases of AVM, 8 cases of AVF, and 2 cases of DAV, due to vasospasm and inadequate diagnosis experience misdiagnosed 1 AVM, due to the limited time resolution and vasospasm missed 2 cases micro-AVM(diameter <1cm) and 1 case of AVF, the sensitivity and specificity of VCTDSA in the detection of intracranial vascular malformation were 100% and 97.8%,respectively. There was no significant difference between VCTDSA and3D-DSA in display the position, size,feeding arteries and draining veins of intracranial vascular malformation; The VCTDSA could find simultaneously cerebral hemorrhage that were not depicted by 3D-DSA in33 cases, and only need one scan to detect the combined aneurysm、feeding arteries and draining veins, but is not sensitive to intracranial vascular steal.CONCULSION :VCTDSA can be used as a sufficient noninvasive methods replacing 3D-DSA for screening and diagnosing intracranial vascular malformations.
