The most important angents are intracranial hypertension, complication and nerve injury. Nowadays therapy for ICH is mainly against intracranial hypertension and complication. Therapy against nerve injury is rare. Hirudin was proved to be able to relieve the thrombase injury in ICH. There is no other therapy being accepted for ICH. The most important reason is that we know little about the pathological process of ICH. There are some reports on the pathological process around the hematoma after ICH. Some signal transduction agents may take part in the pathological process:such as calcium overload, excitatory amino acids, free radical, neurotrophic factor, immune/inflammatic factor, endothelins, calcitonin-gene-related peptide, nitrogen monoxide and β-endorphin,et al. Is the pathological process similar with the ischemic cerebrovascular disease? We know that apoptosis is one of the most important agent in ischemic cerebrovascular disease. We want to investigate the apoptosis occuring in ICH, by which to know something new about the signal transduction agents taking part in the pathological process. Bcl-2 and Caspase-3 are two important and antagonist agents in apoptosis. We want to know the variation of these two agents in ICH and to improve the therapy.Methods:The ICH rat model was made by intracranial injection of Ⅳ collagenase. Healthy male Wistar rats were divided into groups in random. Collagenase/heparin in saline solution or pure saline solution was injected into the caudate nucleus. 6h,12h,24h,3d and 7d after the injection, evaluation of neurologic impairment was made. Then the rats were killed by overdos diethyl ether aspiration. We got the brain frozen sections. The sections were stained by TUNEL to find the apoptosis cell. The expression of bcl-2 and caspase-3 was observed by immunohistochemistry and in situ hybridization. The number and stainingintensity of the positive cells were observed by the image analyzer. Results: 1. After 2ul Collagenase/heparin in saline solution( contain 0.2u/ul Ⅳ collagenase and 2u/ul heparin) was injected into the caudate nucleus, The diameter of hematoma increased in 24h and decreased later. 2. Neurologic impairment with different degree presenced in all ICH rats. The most severe neurologic impairment occurs at 3d after injection of collagenase/heparin in saline solution. 3. Permeation of blood and destroy of structure was found at 6~12h after injection of collagenase/heparin in saline solution. Some TUNEL positive cells were found around the pathway and hematoma after injection of 24h and 3d. The area with TUNEL positive cells at 3d after injection of collagenase/heparin in saline solution is larger than that of 24h(P<0.05). TUNEL positive cells were found around the hematoma and local cortex at 7d after injection of collagenase/heparin in saline solution. 4.The expression of caspase-3 increased significantly after injection of collagenase/heparin in saline solution in 24 hours. The expression of caspase-3 reached the highest level at 3d and decreased later. 5. The expression of bcl-2 immediately decreased at 6h after injection of collagenase/heparin in saline solution. At 12h, the expression of bcl-2 was nearly normal. At 24h, the expression of bcl-2 didn't increase any more. At the same time the number TUNEL positive cells increased in 24 hours.Conclusion: 1. The ideal and reliable cerebral hemorrhage model can be created by local injection of the Ⅳ collagenase. Using heparin, the time of hematoma formation is shortened. 2.Apoptosis exists around the hematoma and cortex. 3.The low expression of bcl-2 is one of the most important agent in apoptosis induced by ICH. 4. The high expression of caspase-3 is one of the most important agent in apoptosis induced by ICH. 5.The apoptosis induced by ICH is related to the expression of bcl-2 and caspase-3.
