To determine the relationship between cytokines in serum and penicillin allergy and the correlation between allergic reaction to penicillin and IL-4, IL-13 candidate gene polymorphisms, we examined the eight kinds of specific IgE antibodies of different antigenic determinants hi serum (BPO-PLL, PVO-PLL, APO-PLL, AXO-PLL, BPA-PLL, PVA-PLL, APA-PLL, AXA-PLL) among 248 patients with allergic reaction to penicillin and 101 control subjects by radioallergosorbent test (RAST), and three kinds of cytokines in serum ( IL-4, IL-13, IFN-y ) of 145 allergic patients and 62 control subjects by enzyme linked immunosorbent assay (ELISA), and 6 candidate loci genie polymorphisms (IL-4 C-589T, IL-13 R130Q, IL-4-IL-13-SNP3, IL-4-IL-13-SNP4, IL-4Ra Q576R, FceIpE237G) of 158 allergic patients and 90 control subjects by PCR-RFLP.1. Associations between penicillin allergy and cytokines like IL-4, IL-13, IFN- 1.1 Associations between penicillin allergy and cytokinesThere were significantly higher IL-4, IL-13, IFN- levels in allergic patients with positive specific IgE antibodies compared to normal controls (P<0.01), and a same result was observed in allergic patients with positive specific IgE antibodies and normal controls compared to allergic patients with negative specific IgE (P<0.01).The result showed that the increase of IL-4, IL-13, IFN- levels may be associated with allergy of positive specific IgE antibodies, and the decrease was associated with allergy of negative specific IgE antibodies.1.2 Associations between the degree of skin test and cytokinesIL-4, IL-13 level appeared a significant difference between positive group and control (P<0.05); this result suggested that IL-4, IL-13 level in serum of patients was growing with the increase of severe degree of skin test.1.3 Associations between specific IgE antibodies and cytokinesAccording to the different number of specific IgE antibodies, the allergic patients with positive specific IgE antibodies were divided into groups with 1, 2, 3 or more positive specific IgE antibodies. Patients with 2 positive specific IgE antibodies had significantly higher IL-4, IL-13 levels than patients with 1 positive specific IgE antibody (P<0.01), and the same result occurs between patients with 3 or more and with 2 positive specific IgE (P<0.01). We found a growing trend of IL-4, IL-13 levels with the number increase of positive specific IgE antibodies.1.4 Associations between specific IgE antibodies and cytokines The correlation between cytokines and specific IgE antibodiesAmong patients with positive specific IgE antibodies, there was a significant association between IL-4 and IL-13 levels (P<0.01), and another association between the two kinds of cytokines and many kinds of specific IgE antibodies (P<0.01). 2. The result suggested that the IL-4Ra Q576R-AA allele was statistically associated with penicillin allergy.2.2 Associations between specific IgE antibodies and gene polymorphismsAmong patients with positive specific IgE antibodies, significantly different distributions of IL-4-IL-13-SNP3, IL-4-IL-13-SNP4, Fcelp E237G genotype were observed between patients with positive BPA and normal controls (P=0.041, P=0.025,P=0.015 respectively). Similar results were observed in IL-4Ra Q576R genotype between patients with positive BPO and controls (P=0.033); in genotype between patients with PVO or APO and controls (7>=0.015, />=0.008 respectively); in IL-4Ra Q576R genotype between patients with positive PVA or APA and controls (P<0.01, P=0.016 respectively).
