Background -Prevention is the best method to treating stroke, and behavior and life Intervention become the important way. During the new times, the occurrence of stroke is rising, and dangerous factors of stroke will change.Purpose- In order to give some guidance to stroke, emotional disturbance, high TC (serum total cholesterol), and abnormal hemorheology were implored as the risk factors of stroke among people in Dalian. The relationship between these factors and stroke was discussed.Methods- This retrospective study selected the cases diagnosed as hemorrhagic stroke or ischemic stroke from Dalian Municipal Central Hospital during 1998-2000 and records of the examiners as the control group in the same time. Main factors such as emotional disturbance, hemorheology, TC (serum total cholesterol) were examined, and other potential confounding factors were age, sex, occupation, histories (hypertension, heart disease, diabetes mellitus) and electrocardiogram changes. Relations between all risk factors and stroke were examined with unconditional multiple Logistic regression model in SPSS 10.0 for considering the adjustment for age, sex and histories of other diseases. Two case-control studies and case-case study were carried out respectively. Statistics method we selected was Backwards: Wald. Differences in the prevalence of all kinds of factors between stroke and free of stroke were tested by Wald test. The items concluded Wald, S.E., Sig. (P values), OR [Exp (B)], 95%C.I.for OR and so on. All probability values were tested with 2-tailed test and P<0.05 was the cutoff indicating statistical significance.Results-among 1190 cases, 670 subjects were ischemic stroke , the other 520 cases were hemorrhagic stroke, and the healthy control group had 450 cases.1. In the study of hemorrhagic stroke, emotional disturbance adjusted OR (odds ratio) was 11.51 (95%CI: 6.51 to 20.36). Abnormal hemorheology OR was 0.07(95%CI : 0.09, 0.54), and TC OR was 0.14(95%CI: 0.39, 0.53), While in the study of ischemic stroke OR of the three factors were 6.19(95%CI: 3.15 to 12.16), 2.17(95%CI to 1.16 to 4.09) and 7.72(95%CI,4.64 to 12.86) respectively.2.Contrast to ischemic stroke in the study of case to case, emotional disturbance OR was 3.03(95.0%C.1: 1.77 to 5.18), On the contrary, the cases with abnormal hemorheology, TC(blood total cholesterol) were inclined to be ischemic stroke, and their OR were 67.40(95.0% C.I.: 26.89 to 168.92), 37.96(95% C.I.: 19.05 to 75.65).Conclusion-The occurrence of stroke had close relation to emotional disturbance, TC level, and abnormal hemorheology. Emotional disturbance was the risk factor for stroke, especially for hemorrhagic stroke, and TC level, abnormal hemorheology were only the risk factors for ischemic stroke. So it is necessary to implement a comprehensive prevention program, such as lifestyle and dietary habit modification, and good mentality maintenance. |
