Background and Objective: Spontaneous intracerebral hemorrhage (ICH) accounts for 10% to 20% of all strokes and is associated with extremely high morbidity and mortality. How to reduce the mortality and mutilation rate; and to improve life quality have been pay highly attention. However, there is no effective medical treatment for ICH, and the value of surgical treatment is still controversial. A poor understanding for pathophysiological process of neuronal injury after ICH is responsible for the lack of effective therapy. In the past, neuronal injury surrounding the hematoma after ICH was thought to be due to be bleeding and compression from hematoma. Now investigation found that brain secondary injury of hemorrhage may be more important. It included cerebral edema, destruction of blood-brain barrier (BBB), ischemia and free radial lesion around hematoma, and et al. Inflammatory mechanism has been highly thought to play an important role in hemorrhagic brain injury.Interleukin-1β(IL-1β) is an inflammatory medium posses multi-tropism biological function. It regulated inflammatory and immunologic reaction, which closely correlated with neuron injury. Brain Levels of IL-1βincreased after ischemia. Intracerebral adminstration of IL-1β induced inflammation, cells death and vasogeic edema in brain. Overexpression of IL-1 receptor antagonist attenuated brain edema formation and thrombin-induced intracerebral inflammation following ICH.P-Selectin stored in macrophage, α-granule of platelet and Weibel-Palade Corpuscle of endothelial cell of blood vessel. It serves as an adhesion receptor mediating the binding of platelets to leukocytes, and took part in the process of rolling, attaching, adhesion and swimming chemotaxis of leukocyte in the illness procedure. P-selectin and CD63 is expressed exclusively on platelet activation. A few researches have shown that levels of P-selectin evated in serum of patient with stroke.S-100 protein is considered as specific markers of brain damage after stroke, trauma, and other injuries. Serum concentration of the protein was correlated with the size of infracted area of Post-stroke brain. Investigation showed that serum level of S-100 protein was also elevated significantly in patients with ICH. S-100 protein level was also found being associated with clinical outcome.The purpose of this study was: ①to explore the pathophysiology of hemorrhagic braininjury by observing the activity changes of IL-1β and P-selectin in serum and CSF in patients with ICH; ②to study the correlation between activity changes of inflammatory factors and hemorrhagic brain injury.Methods:1 Case grouping:1.1 ICH group: 53 cases, 34 Male and 29 Female. Age 60.8±12.9(28~83) years old. Which is from inpatients of Department of Neurology, Daping Hospital. Diagnosed by case history, physical examination, CSF and head CT(or MRI). Coincidence of "Diagnosis Standard of the Fourth National Academic conference of Cerebral Vascular Disorders(1995)". 1.2 Control group: 25 Cases. 10 Male and 15 Female. Age 48.3±15.3(18~74) years old. Which is also from inpatients of synchronization case. Except for organic diseases in central nervous system.2 Degree of illness: Accessing by "Score Standard of Neurologic Impairment Degree" to mild, middle and Severe Degree.3 Collection and determination of Samples.3.1 Sample collection: to draw elbow venous blood 4ml at 7:00am on the next day, and to collect CSF 2ml at 9:00am by lumbar puncture on the same day.Collect blood and CSF 2 times in acute phase(<7d) and recovery phase(>21d).3.2 Determination of IL-1β, Ps and S-100 Protein: by enzyme linked immunosorbent assay (ELISA).4 Statistical analysis: results were expressed by Mean±Standard deviation(±s).
