Image Manifestation Of Bronchial Arteriography,Pathologic Mechanism And Results Of Bronchial Artery Embolization For Massive Occult Hemoptysis

Objective: To clarity the image manifestation of BAG(bronchial arteriography) , pathologic mechanism and results of BAE(bronchial artery embolization) for the massive occult hemoptysis .Materials and Methods: The massive occult hemoptysis were defined that chest radiograph and/or CT(computed tomography) were negative or some lesions showed by these image examinations were not the sites of hemoptysis proved by BAG and can not explain the signs of blooding. 32 patients of the massive occult hemoptysis were performed BAG to observe abnormal phenomena of bronchial artery and 25 patients were performed BAE to observe the immediate effects and long-term results of nonrecurrence.Results: All patients' BAG showed abnormal vessels, included different degree hypervascularity, proliferation and derangement. Serious cases were accompanied by blooding stains in the segment or lobe. Some patients have broncho梐rterial fistula or arteriovenous macformation, that was identified by pathologic mechanism of 7 patients' surgical operation. 25 patients were operated with BAE, their immediate effects and long梩erm results were very paramount.Conclusion: Pathologic mechanism of the massive occult hemoptysis is the damage of bronchial artery include enlargement, hpervascularity and proliferation ,or enlargement like aneurysm ,the wall of bronchial artery become unregular thick, which are mainly caused by bronchitis and infection and some caused by bronchial arterialdeformity. BAG and BAE are proven to be effective in the diagnosis search and therapy and lack in the mortality and mobility.
