Objective: To investigate the changes of the serum levels of interleukin-6(IL-6), C-reactive protein(CRP) and tumor necrosis factor- a (TNF- a ) in the unstable angina pectoris(UAP) and to assess the significance in the dangerous classification and the prognostic value of UAP.Methods: 1, The serum levels of IL-6, CRP, TNF- a , creatin kinase-MB(CK-MB) and cardiac tropnin-I(cTn-I) were measured in the groups of UAP, stable angina pectoris(SAP) and healthy control and were compared within the three groups. IL-6, TNF- a were measured with enzyme-linked immunosorbent assay, CRP was measured with rate nephelometry immunoassay. 2, To classify UAP as the higher, middle and lower dangerous groups and to compare the serum levels of IL-6, CRP, TNF- a CK-MB and cTn-I within the three groups. 3, To assay the correlation within IL-6, CRP, TNF- a , CK-MB and cTn-I in UAP with linger regression. 4, To compare the serum levels of IL-6, CRP, TNF- a , CK-MB and cTn-I in UAP before and after treatment. 5, To follow up the group of UAP for 3-6 months.Results: 1, The serum levels of IL-6, CRP, TNF- a , cTn-I in UAP are apparent higher than SAP and control (p<0.01), The serum levels of CK-MB are apparent higher than control (p<0.05). 2, The serum levels of IL-6, CRP, TNF- a , cTn-I, CK-MB in higher dangerous group in UAP are apparent higher than in middle dangerous group and lower group(p<0.01). The serum levels of IL-6, CRP in middle dangerous group in UAP are higher than in lower dangerous group(p<0.05) and there are no difference in the TNF- a , cTn-I and CK-MB(p>0.05). 3, There are positive correlation between IL-6 and CRP in UAP(r=0.38, p<0.05). And a apparent positive correlation between IL-6 and CK-MB(r=0.69, p<0.01). There are no correlation in other factors(p>0.05). 4, In UAP, the serum levels of IL-6, CRP, TNF- a , CK-MB were reduced after treatment (p<0.05-0.01) and the cTn-I was also reduced but there are no statistical significance. 5, In the patients suffered cardiac events again during the follow-up time of 3-6 months, the the serum levels of IL-6, CRP, CK-MB were higher than the other patients(p<0.05) and the TNF- a , cTn-I were higher as well, but there are no statistical significance.Conclusions: 1, IL-6, CRP, TNF-a and cTn-I in unstable angina pectoris are apparent higher than in stable angina pectoris. 2, The serum levels of IL-6, CRP, TNF-a, CK-MB and cTn-I are valuable in the dangerous classification of unstable angina pectoris. 3, The serum levels of IL-6, CRP and CK-MB are valuable to assess the prognosis of the unstable angina pectoris.
