Objective (1) Cases of fracture of femoral neck treated with artificial femoral head replacement were studied and the acetabular wear by the metallic femoral head were measured. (2) Bone mineral density(BMD) was measured in those old patients with hip fracture, to analyze the cause and characteristic of hip fracture.Materials and methods (1) To measure the acetabular wear with special computer soft ware program for 84 cases of femoral head replacement arthroplasties 1-6 years after the operation and to analyze the radiological acetabular wear with the clinical results. (2) To measure the BMD in those old patients with hip fracture, to compare the difference with BMD in those male and female fracture patients and to analyze the cause.Results (l)Hip pain occurred in 78% of 84 cases on an average of 3.8 years after the femoral head replacements, the wear rate was 1.06mm/year. (2)Classified them with the same age, compared them with BMD in the patients with hip fracture over 60 years, there was significant difference in BMD amongthe 60 years group and 70 years group with hip fracture(p<0.01) , but there was no significant difference in 80 years group's(p>0.01), the value of BMD in the old males with same age is higher than that in the females, the peak value of BMD in females was low and the bone deficit was serious . When the age increased, the value of BMD in old peoples was falling, the females were obvious especially.Conclusions We propose that total hip arthroplasty may be superior to femoral head replacement for patients with femoral neck fracture in old aged. Osteoporosis is the main inner factor causing hip fracture, the extent of osteoporosis in female patients is much more serious than that in male patients. That the level of estrin falling in those postmenopausal female patients is an important influential factor which leads to that the extent of osteoporosis is more serious.
