Forearm intramedullary nails included Rush pins, Street pins, Sage pins, and so on .All of these nails might exit from the entry, fail to control the fracture rotation.We developed a kind of forearm interlocking intramedullary nail. Based on the related study of biomechanics and anatomy, the nails were used to treat patients.A Comparative Biomechanical Study about Interlocking Intramedullary Nails and Four Kinds of Other Fixation Devices in Treatment of Forearm FracturesObjective: The biomechanical function about forearm fractures fixed by interlocking intramedullary nails was tested and compared with four kinds of other fixation devices, which provided the theoretical basis before nails were put into clinical practice.Methods: The intensity and rigidity of forearm fractures fixed by interlocking intramedullary nails was compared with that of fractures fixed by four kinds of other fixation devices in axial compress test, bend and torsion test. In the experiment, 15 pairs of radii and ulnas were used. Four kinds of other fixation devices were as follows: six-hole dynamic compression plates, semi-tubular plates, external fixation devices, triangular pins.Results: Compared with four kinds of other fixation devices, The force of anti-compress ,anti-bend and anti-torsion of forearm interlocking intramedullary nails was more powerful.Conclusion: The forearm interlocking intramedullary nails had excellentbiomechanical function. It is good to exercise early ,restore the function of forearm and get fracture healing after operation.2. The Related Anatomical Study about Deep Branch of Radial Nerve and Interlocking Intramedullary Nails in Treatment of Radial fracturesObjective: To provide anatomical basis for the use of radial interlocking intramedully nails and avoid injuring the deep branch of radial nerve.Methods: The deep branch of radial nerve were dissected, which based on 22 pieces of fresh forearm specimens. Joining the lateral epicondyle of humerus and Lister tubercle of radius, the line passed through 6 horizontal planes, which were as follows in sequence: 0, 1 .0cm, 1.5cm, 2.0cm lower than the articular of radial head respectively, the horizontal planes of deep branch of radial nerve entering the supinator and winding across the radius. And the crossed points were named point A, B, C, D, E, F accordingly .The distance between deep branch of radial nerve and fixed points B, C, D was measured separately when the forearm was kept in pronation, neutral and supination.Results: The distance between deep branch of radial nerve and fixed points B, C, D became nearer and nearer in the sequence of B, C, D. With the forearm in pronation, neutral and supination sequently, every fixed point became farther and farther from the deep branch of radial nerve. Both the distance from point A to E and from point A to F were measured.Conclusion: When interlocking intramedullary nail of radius was used to treat the radial fracture, it was relatively safe to keep the forearm in neutral, drill and insert the distal locking nail on point B from the posterolateral to anteromedial.3. The Design and Clinical Application of Interlocking Intramedullary Nails in the Treatment of Forearm FracturesObjective: A kind of forearm interlocking intramedulary nail is to be designed and put into clinical practice after some related experiments. The effects will be evaluated. Methods: The forearm interlocking intramedullary nails were used to treat 14 casesof forearm fractures. There were 11 cases of fresh fractures, 1 case of old fracture, 2 cases of nonunion.Results: There is no infection of the wound, no difficulty in forearm rotation. One of the 14 patients was lost to follow up. The other 13 patients were followed up varying from 7 weeks to 8 months after operation. 7 cases obtained solid union, 3 cases got clinical union, and 3 cases shew obvious porosis.Conclusion: Forearm interlocking intramedullary nail was a kind of strong and elastic fixation.
