Objective To explore the effect of Shenfu Injection on experimental reperfusion injury of extremities. Meththod Sixteen healthy male rabbits were divided into two groups randomly. The inferior extremity of abdominal aorta was blocked with vascular clamp to result in hindlimbs ischemia for 5 hours, then the vascular clamp was moved and Shenfu Injection 2ml/kg (experiment group) or equivalent saline(control group) was injected immediately through the lumen of the abdominal aorta. After 24 hours' reperfusion, the venous blood samples were taken from inferior vena cava to measure the levels of blood serum Creatinine (Cr),Urea,Kalium (K+) and Creatine Kinase(CK). Tibialis anterior muscles and sciatic nerves were collected to make into pathological slices(HE-staining) and ultrathin-section. Two pathological doctors were invited to read the skeletal muscle pathological sections with light microscope respectively. Each of them rank the sections from 1 to 16 according to the degree of muscle injury (from the gentle to the severe), take the average value of each sample's two ranks as each sample's final rank. The ultrathin-sections of skeletal muscle and nerve samples were taken for observation with transmission electron microscope. Results After 24 hours' reperfusion,the levels of the biochemical markers were as below(the experiment group/the control group): K+(mmol/L)5.4±0.53/6.8±0.59,Cr(μmol/L)154±31/307±34,Urea(mmol/L)18.4±2.2/24.7±2. 7,CK(U/L)54463±19990/111502±21401. Statistical analysis (T-test) shows that the levels of the experiment group were lower than the control group (P<0.001). The skeletal muscle samples' final ranks were 3.0,3.5,6.5,3.5,4.0,5.5,10.0,3.5(experiment group) and 15.0,12.0,14.5,12.5,9.0,13.0,7.0,13.5( control group). Statistical analysis (rank sum test) shows that the results of the two groups are different (P<0.01). Myofilament lysis, severe edema of mitochondria, dilataltion of sarcoplasmic reticulum, endothelial cells' notable swelling and basement membrane rupture of microvessel, interstitial edema and leukocytic infiltrate can be seen in the control group; Myelin sheaths' dissociation and axioplasm atrophy of the myelinated nerve fibers also can be observed. All these injuries are not so severe in the experiment group. Conclusion The results of this experiment show that the Shenfu Injection has therapeutical effect on experimental ischemia-reperfusion injury of extremities. It's a better choice of clinical drug therapy for reperfusion injury of extremities.
