ObjectiveIn China, Hepatitis B is prevalent. The chronicity of HBV infection is mainly caused by cellular immune deficiency . Peripheral T cell subset is a vital factor to indicate human immuno state. At present, some studies have demonstrated an association between chronic hepatitis and cellular immune. But the result were inconsistent. This study was to determine the changes of the peripheral CD3+ CD8 + ,CD3 + CD4 + ,CD3 + in different study groups using flow cytometry.SubjectsA total of 42 subjects were divided into 3 groups: control group ( 10 heathy subjects) , HBV carriers group (11 cases, from Shenyang Infections Diseases Hospital ) , chronic hepatitis B group (21 cases who were in - patients from Shenyang Infections Diseases Hospital ).MethodsCD3 + CD4 + ,CD3 + CD8 + ,CD3 + lymphocyte were assayed for all study subjects using flow cytometry.Result1. The ratio of percentages of CD3 + CD4 V CD3 + CD8+ decreased and the percentages of CD3 + CD8 + increased significantly differenance in CHB group compared with the control group.2. There were no differences in the ratio of percentages of CD3 + CD4V CD3 + CD8 + and the percentages of CD3 + CD8 + between ASC and control group.3. When comparisons were performed among 3 groups, the control group had the highest ratio of the percentages of CD3 + CD4+/ CD3 + CD8 + the CHB group had the lowest. The CHB group had the highest percentages of CD3 + CD8 + , the control group had the lowest. The differences among 3 groups reached statistic significance.4. There were no differences in percentages of CD3 + CD4 + and CD3 in 3 study groups.DISCUSSIONHepatitis B is a kind of immune - related disease . There is no direct hepa-tocyte damage after HBV infection. It's a indirect damage through inducing im-munological reactions. After the body infected by hepatitis B virus,the balance in the peripheral blood lymphocyte subsets is broken, so that caused changes of the percentage of lymphocyte subsets,the total cell amounts and cell functions. These changes can indicate the body imuno - state . The imuno - mechanism of CHB is complex. The changes of the peripheral blood lymph subsets has not yet cleari-fied. After HBV infection, the immuno response to viral antigen are associated with viral clearance and pathogenesis. It includes specific CTL,CD4 + Th cell, nonspecific monophage cell and NK cell, which can induce direct hepatocyte damage or cause the demage through cytokines. The immunopathologic of hepatitis B was mainly determined by the T cell immuno response. After CTL cell recognizing target antigen in the hepatocyte surface , its 2 functions will conduct: kill a small part of infected hepatocyte ; clear the virus without cytopathy through secreting IFN - and TNF - . CD4 + T cell can be divided into 2 types ,Thl and Th2 cell. The balance between Thl and Th2 cell might affct the level of HBV clearance and the severity of the hepatocyte damage . The imbalance between Thl and Th2 cell will play a crucial role in pathogenesis of chronic hepatitis B.The results showed that the ratio of the percentages of CD3 + CD4 V CD3 +CD8+ decreased and the percentages of CD3 + CD8 + increased significantly in CHB group compared with control group . There were no difference between ASC and control group . There were no differences in the percentages of CD3+ CD4+ , CD3 + among 3 study groups. The ratio of the percentages of CD3 + CD4 +/CD3 +CD8 + decreased and the percentages of CD3 +CD8 + increased significantly among these 3 groups.These results are accordant with other researchers findings. On the other hand,some inconsistent results were also observe which showed the percentages of CD3 + CD4 + was lower in CHB subjects than the control group. There were no differences in percentages of CD3+ among CHB, AsC and healthy control groups. This finding suggested that no change in the amount of mature T cell in patients infectedby HBV.
