Objective: To investigate the relations between Helicobacter pylori (Hp) infection and coronary artery disease (CAD) as well as the effects of Hp on endothelial function, lipid metabolism, serum immunology, folate, Vitamin B12 and Homocysteine. And thus, further understand the mechanism of Hp infection on the progression of CAD. Methods: HpIgG (ELISA) and thromboxane B2 (TXB2), prostacyclin I2 (PGI2),Nitric Oxide (NO), interleukin-1 (IL-1 ), folate, VitB12, Homocysteine (Hcy) (RIA) and total cholesterol (TC), triglyceride (TG), apolipoprotein A (ApoA), apolipoprotein B (ApoB), high density lipoprotein (HDL), low density lipoprotein (LDL), blood sugar, white blood cell count (WBC) were measured in 242 cases of CAD, with 88 subjects without CAD as controls, and risk factors were inquired in CAD group. HpIgG was measured in both total 242 cases with CAD and 88 subjects without CAD and compared between these two groups. And experimental data mentioned above were compared and analysed between HpIgG positive group and HpIgG negative group in the patients with CAD. Refutes: (1) The rate of seropositive for HpIgG in CAD patients is significantly higher than that of controls (53.3% VS 38.6%, P<0.05). (2) For the cases of CAD, the serum HDL level in HpIgG seropositive group is significantly lower than that of HpIgG seronegative one(P<0.05)and the serum Hcy level in HpIgG seropositive group is significantly higher than that of HpIgG seronegative one(P<0.05),respectively. No correlation, however, was found between Hp infection and essential hypertension, diabetes mellitus, tobacco use, age, gender and alcohol consumption. (3) For the cases of CAD patients, the level of plasma TXB2 in group with HpIgG seropositivity is markedly higher than that in group with HpIgG seronegativity(P<0.01)and the level of plasma PGF in these of HpIgG seropositivity is lower obviously than that of HpIgG seronegativity (P<0.05).Conclusion: Hp infection may be associated with CAD. It may lower serum HDL level and increase serum Hcy level, and thus, can accelerate the progression of CAD.Theplasma TXB2 is higher and PGF is lower in CAD patients with HpIgG seronegativity, which may be one of pathogenic mechanisms of Hp infection in coronary heart disease.
