Objective The plasma NO and ET of the perioperative period is detected in the cirrhotic patients with portal hypertension after splenectomy and portal-azygous disconnection, and compared with the normal contrast. We studies the mechanism of NO and ET in the patients with the portal hypertension, and studies scientitive the portal-azygous disconnection in the surgical methods and afford methods for the clinical works.Methods We collected 40 patients with the hepatitic cirhosis after hepatitis B,These patients have divided into two groups.20 patients with the portal hypertension, 20 patients with the hepatitic cirhosis .compared with 15 patients with nonhepatic disorders. We collected 4ml blood via lubital vein seperately 48h before bootal-azygous disconnection, 24h afear the surgery, 72h after the surgery and 7days in portal hypertension group and collected 4ml blood in normal group and hepatitic cirhosis group. Blood was detected NO metabol itant, [NO3-][NO2-] by using the radroimmunoassay.Results Compared with normal group(33.63 4.22) moul/1 and hepatitic cirhosis group(45.73 8.25) mol/1, cubital venous plasma NO levels significantly increased in portal hypertension group patients (97.23 10.83) mol/1 before surgery, P<0.01 and P<0.05 seperately. Compared with normal group , cubital venous plasma NO levels significantly increased in portal hypertension group after surgery P<0.01. Comparation before and after surgery had significantly difference in portal hypertension group P<0.05 and we found the decreasion of plasma NO levels in proportion to the decreasion of the portal pression (rs=0.422, P=0.063).Compared with normal group (12.96+8.32) pg/ml and hepatitic cirhosis group (26.46 9.65 ) pg/ml, the plasma ET levels significantly increased in portal hypertension group (58.44 ?17.45 ) pg/ml before surgery, P<0.01 and P<0.05 seperately. Compared with normal group , plasma ET levels significantly increased in portal hypertension group after surgery P<0.01. Companation before and after surgery had significantly difference in portal hypertension group P<0.05. And we found the decreasion of plasma ET levels in proportion to the deereasion of the portal pression(rs=0.61,p=0.034).Conclusion The plasma NO and ET have significantly in fluence on the pormation of the portal hypertension, the increasion of the plasma NO and ET levels has significantly in fluence on the portal vascular nesistance. The splenectonry and portal-azygous disconnection can decrease the pression of the portal vein and can decrease the plasma NO and ET levels.
