| Objective To study the epidemic characteristics of hip fracture and analyze the epidemiology of cervical and irochanteric hip fractures in the two provincial hospitals of Hefei from 1998 to 2002. Methods The cases of hip fractures in Anhui Provincial Hospital and the First Affiliated Hospital of Anhui Medical University were recorded from 1998 to 2002 considered in our study. Questionnaires were devised to fill in the corresponding information and the trends of hip fracture were analyzed by linear regression model. Epidemiologic features and correlation factors were compared and analyzed between cervical and trochanteric hip fractures. Results 1058 cases (526 for males and 532 for females) were collected , femur neck fractures were 667 and intertrochanteric fractures were 391, respectively, the left of hip tends to be more breakable than the right. The old people were susceptible to hip fractures mostly. The patients showed an avarage age of 61.69 years old. The incidence of both transcervical and pertrochanteric hip fractures increased significantly among people over 60 years. The difference in general distribution was not obvious between male and female, but male cases predominated in the cases before the age of 50, and women aged above 50 years old suffered from the hip fracture were more than those of men. The mean age of all intertrochanteric fractures was 3.8 years older than that of femoral neck fractures. It was more common in those who engaged in manual labor than those who in brainwork. Slight injures were the major causes of hip fractures. 777 cases were caused by slipping up or/and wrenching, accounting for 73.44% of its total. Trend analysis showed that hip fracture presented an upward trend as the age growing, and got to its climax between age group 60 and 85. we also found that cases of males were in increased trend during last five years, with annual 9.68% (P< 0. 05) and cases of females showed a slightly decreasing trend ,withannual 0.75%.but no significant difference were found (P>0. 05). Hip fractures have a relationship with seasons and occur more common in winter. The incidence of cervical fracture was higher than that of trochanteric fracture. More intertrochanteric fractures occurred in men (The ratio of male to female cases was 1.44:1) and much more cervical fractures occurred in women (Female /male ratio was 1:0.79). A pattern of rapidly rising rates with age was seen for cervical and trochanteric hip fractures among postmenopausal women. 87.6 percent of all female cases occurred within 30 years after menopause. Most of these kinds of subjects were senile and complicated with medical diseases. The fundamental treatment for hip fracture was operation. Conclusions Hip fracture in Hefei provincial hospital showed an upward trend during 1998-2002, and the treatment and prevention of the hip fractures among middle-aged and elderly population should be stressed, especially avoid falling, and prevent and treat osteoporosis. |
