Objective:To compare the efficacy of the treatment of unstable intertrochanteric fracture of femur with dynamic condyler screw(DCS) and proximal femoral nail(PFN),and to discuss the most idealistic immobilization methods to supply the clinical basis for the further biomechanics study.Methods:From February 2006 to January 2007,60 cases of unstable intertrochanteric fracture of femur were treated in our hospital(Zhangzhou hospital of TCM),who accorded with subsuming criterion.All the cases were divided into two groups randomly with the random number table.Then one group of 30 cases were treated by DCs,the other were treated by PFN. The common conditions,operative procedures,postoperative recovery and postoperative complications of the two groups were evaluated and compared.Results:All the patients were followed.The operation time of DCS group was 79.80±7.05 minutes and the operation time of DCS group was 86.40±16.85 minutes,so there was no significant difference at the operation length between the two groups(P>0.05).But there was obvious significant difference in the length of incision,hemorrhage,times of fluoroscopy and drainage(P<0.01).Neck-shaft angle of one year after operation:DCS group was 125.30°±7.85°,and PFN group was 127.27°±4.82°,so there was no significant difference between two groups(P>0.05).Harris hip score of one year after operation:DCS group was 83.80±9.00,and PFN group was 88.07±6.36,so there was significant difference between two groups(P<0.05).Postoperative complications:One patient appeared coax vara in PFN group, and four in DCS group.But the differences in postoperative complications between the two groups were no statistical significance(P>0.05).Conclusion:1.The complications and Neck-shaft angle of one year after operation with internal immobilization of PFN is resembled with DCS in treating unstable intertrochanteric fracture.2.The internal immobilization of PFN is much better than DCS in Operation invasion and postoperative function recovery.
