Objective:To explore the mechanism and clinical significance of TRAIL-induced apoptosis in Hepatocellular carcinoma, this thesis investigated the expression of DR4,DR5,DcR1 and DcR2 mRNA,the main recepters of the TRAIL, in liver tissue of patients with HHC. Methods: DR4,DR5,DcR1 and DcR2 mRNA expression and its level were assessed in 23 cases of normal liver tissue,40 cases of para-liver cancer tissuse and 40 cases of hepatocellular carcinoma tissue with semi-quantitative reverse transcription polymerase chain reation (RT-PCR) procedure.40 cases of hepatocellular carcinoma tissue were applied to detect the expression of P53,P21,nm23 and VEGF with immunohistochemical method .Results: ⑴ There was no difference expression rate of DR4 mRNA in the normal liver tissue, para-liver cancer tissuse and hepatocellular carcinoma tissue (89.2%±2.1%,91.3%±2.5%,94.6%2.4%,p >0.05);(2) There was no difference expression rate of DR5 mRNA in the normal liver tissue, para-liver cancer tissuse and hepatocellular carcinoma tissue (88.6%±2.7%,93.3%±1.8%,96.3%±1.3%, p >0.05);(3)The positive expression rate of DcR1 mRNA in the normal liver tissue was significantly higher than hepatocellular carcinoma tissue (86.3%±±1.3%,23.5%±3.5% ,respectively, p <0.05). The positive expression rate of DcR1 mRNA in para-liver cancer tissuse was significantly higher than hepatocellular carcinoma tissue (76.2%±2.1%,23.5%±3.5% ,respectively, p <0.05).There was no difference expression rate of DcR1 mRNA between the normal live tissue and para-cancer liver tissue. (86.3%±1.3%,76.2%±2.1%,respectively, p >.05);(4) The positive expression rate of DcR2 mRNA in the normal liver tissue was significantly higher than hepatocellular carcinoma tissue (76.8%±3.2% , 15.5%±2.5%,respectively, p <0.05). The positive expression rate of DcR2 mRNA in para-liver cancer tissuse was significantly higher than hepatocellular carcinoma tissue(64.3%±4.4%,15.5%±2.5%, ,respectively, p <0.05),There was no difference expression rate of DcR2 mRNA between the normal live tissue and para-cancer liver tissue. (76.8%±3.2%,64.3%±4.4%,respectively, p >0.05);(5)There was no difference expression average level of DR4 mRNA and DR5 mRNA in the normal liver tissue, para-liver cancer tissuse and hepatocellular carcinoma tissue (p =0.327,p =0.264);(6)The expression average level of DcR1 mRNA and DcR2 mRNA in the normal liver tissues, para-liver cancer tissuse was significantly higher than hepatocellular carcinoma tissue(p <0.05), There was no difference expression average level of DcR1 mRNA and DcR2 mRNA between the normal liver tissue and para-liver cancer tissuse (p =0.379);(7)Statistical correlation among expression change of DR4 or DR5 mRNA and patient's age, size and differentiation of the tumor, AFP level of the patient's serum and HBsAg was found (p <0.05). (8) There was no difference expression average level of DR4 and DR5 mRNA between P53(+)/P21(+) hepatocellular carcinoma tissue and P53(-)/P21(-) hepatocellular carcinoma tissue ,(p >0.05) ; the expression average level of DR4 and DR5 mRNA in the hepatocellular carcinoma of nm23(+) was significantly higher than that of nm23(-),( p <0.05); the expression average level of DR4 and DR5 mRNA in the hepatocellular carcinoma of VEGF(+) are significantly more lower than that of VEGF(-),( p <0.05).Concludes: (1) There was no difference expression rate of DR4 and DR5 inthe normal liver tissue,para—liver cancer tissuse and hepatocellular carcinomatissue;(2)The expression of DcRl and DcR2 in the normal liver tissue orpara—liver cancer tissuse was higher than those in hepatocellular carcinoma tissue,which indicated that death recepter and decoy recepter differented expression in liver tissue may be a important mechanism in the TRAIL-induced apoptosis.(3)The expression of DR4 and DR5 was good correlated with HCC pathologicalstatus,which indicated that it would be limite that we used TRAIL to treat HCConly.We may use I TRAIL wth other therapeutics to...
