Premature rupture of membrane(sPROM) refers to pregnancy over 37 weeks after membrane rupture before labor . Preterm premature rupture of membranes (P-PROM) refers to dissatisfaction with premature rupture of membranes occurred at 37 weeks of pregnancy. The incidence of PROM was 10%; P-PROM the incidence is 2 to 3.5% with 30% ~ 40% of premature, while 85% of neonatal morbidity and mortality is caused by P-PROM .After membrane rupture, neonatal asphyxia, neonatal pneumonia, neonatal sepsis and other serious complications are inclined to happen. It can endanger the safety and health of mothers and infants without proper handling. In order to reduce maternal and fetal mortality and improve the quality of newborn babies, People pay more attention to it. Studies have shown a large number of matrix metalloproteinases ( MMPs) families are able to degrade extracellular matrix ( ECM) of all the protein constituents of endogenous enzymes. Extracellular matrix of the corresponding components of the degradation which may control content and membrane structure can weaken membrane and cause the occurrence of premature rupture of membranes. Recently a number of experiments have found there were the increasing of MMPs and the reduction of tissue inhibitor matrix metalloproteinase (TIMPs) in the membrane of PROM.Objective:To investigate the expression of the matrix metalloproteinase -2 and -2 inhibitors in fetal membrane in premature rupture of membranes in pregnant women and it's relationship with premature rupture membranes.Methods:Collecting 30 cases of confirmed cases of premature rupture of membranes (PROM group), normal pregnancy in the same period of selective collection of 30 cases of cesarean section (control group) at Obstetrics and Gynecology of the First Hospital of Shan Xi Medical University, in April 2007 ~ December 2007. The ages and gestational age of two groups show no significant difference .The rupture of membrane happened within 24h .The two groups had no maternal diseases, other obstetric complications, non-production trillion, non-cervical dilatation, early maternal, single-head bit. Membrane's collection: To cut size of 2cm×2cm fetal membranes after surgery (to retain fetal membranes with a natural break of premature rupture of membranes), the saline washing, the conventional 10% solution of formaldehyde fixed 24-48 hours. We used immunohistochemistry PV9000 method to detect the MMP-2 and TIMP-2's expression in fetal membranes.Results:1. Compared with the control group, the incidence of mild infection and severe infection in Premature rupture of membranes group has notably increased . There is statistical significance with the fisher exact probability P = 0.043.2. The expressions of matrix metalloproteinases-2 in premature rupture of membranes are significantly higher than that in control group (P <0.01). Positive expressions are 100% and 46.67%. There is statistical significance (P <0.01).3. The expressions of Inhibitor matrix metalloproteinase-2 are lower than normal pregnant women group, but the difference is not significant. There is no statistical significance (P> 0.05).Conclusion:1. There is closely relationship between moderate and severe chorioamnionitis and Premature rupture of membranes.2. The expression of MMP-2 rise in membrane or the imbalance expression of matrix metalloproteinase -2 and -2 inhibitory factor in the fetal membrane may be one of the reasons that caused premature rupture of membranes.
