Object To explore the effect of interleukin-6(IL-6) and tumor necrosis factor alpha(TNF- ) on the pathogenesis of postmenopausal osteoporosis(PMOP) and the changes of IL-6, TNF- after administration of risedronate. Methods (1)According to the result of bone mineral density(BMD),76 postmenopausal women were allocated to three groups. 33 cases of PMOP were assigned to Group C, 19 cases of postmenopausal women without osteoporosis to Group B and20 cases of healthy premenopausal women to group A. Serum levels of lL-6, TNF- , estrogen(E2) and osteocalin (BGP)were detected by radiotmmunoassay method.(2)46 PMOP and osteopenia women were randomly divided into risedronate and control groups. The twenty-three women in risedronate group were orally given risedronate 5mg every morning and Calcichew D (consisted of calcium carbonate 1250mg and vitaminD3200IU) one tablet every night. The twenty-tree women of control group were taken placebo 5mg every moring and Calcichew D one tablet every night. The duration of the treatment was 12 months .Before and after treatment, serum levels of lL-6> TNF-a , BGP were detected by radioimmunoassay method, and urine N-terminal telopeptide of type 1 collagen(NTX) by ELASA method. Results (l)Serum levels of IL-6, TNF- a were higher in GroupC and groupB than in Group A,and highter in GroupC than in groupB; Serum level of BGP and the ratio of urine NTX to urine creatinine (NTX/Cr) were higher in GroupC and groupB than in Group A,and highter in GroupC than in groupB.A correlation analysis in all women showed the IL-6, TNF- levels were correlated negatively with BMD, Positively with BGP.(2)In residronate group,serum levelsof IL-6, TNF-a , BGP and the ratio of urine NTX/Cr were decreased and BMD were increased after treatment. Compaired with control group there were significiant differences. While the control group had no significiant differece after treatment. Conclusion (l)The levels of IL-6 and TNF- are high in PMOP woman (2)Risedronate prevent bone loss in PMOP, the mechanisms of it is at least in part by inhibiting IL-6 and TNF-a production.
