BackgroundLumbar disc herniation is one of the most common causes of low back pain,it isgenerated based on lumbar intervertebral disc, the etiology is still unclear. The domesticand foreign scholars have conducted in-depth research from different acpects, includingnutrition, biomechanics, cell factor, gene. In recent years,tumor necrosis factor (TNF)and its receptors take part in disease process,such as digestive, respiratory, endocrine,gynecological, the study is relatively less on the effect of degenerative intervertebral disctissue.ObjectiveTo explore the correlation of TNF, TNFR, sTNFRS changes in degenerative intervertebral disc tissue,Correlated with the pain of sciatic nerve and the relationship withdegeneration.Research methodsThe grouping of selected specimens: the experimental group contain40cases, Oncondition that were the first onset of protrusion of the intervertebral disc operation patients,aged between18---60; the control group contain10cases, from no protrusion of theintervertebral disc by injury to lumbar burst fracture patients in need of decompressionoperation, between the age20--40years old, selected specimens containing differentproportions of fiber nuclei, nucleus and a small amount of cartilage endplate. Applicationof enzyme-linked immunosorbent assay (ELISA) to detect the concentration of TNF-α,TNF-β and sTNFR11, sTNFR2; Reverset ranscription polymerase chain reaction (RT-PCR)method was used to detect the expression of the target cell membrane surface TNFR1/R2,mrna. Then the data is processed with SPSS16.0statistical software, calculate the indexstandard deviation and mean, analysis of the overall difference and the difference betweengroups. The ill group content of TNF-αand clinical comparative analysis of the results, and statistical analysis using statistical softwa.ResultsDisease group and control group in the determination of TNFα/βand sTNFR1/R2exist significant difference; In patients with clinical signs and symptoms score results andoutstanding intervertebral disc tissue tumor necrosis factor and its receptor expressed bythe positive correlation; the control group in the expression of TNFRl/R2mrna wassignificantly lower than the prevalence of expression group. There also exists a certaincorrelation between the prevalence of group in herniated disc tissue TNF-α/βsTNFRl/R2and TNFRl/R2mRNA.Conclusion1) This experiment through the establishment of the standard curve of tumor necrosisfactor and its receptor, By double antibody clip method to detect concentration of TNF-α,TNF-β,TNFR1and sTNFR2.2)Reverse transcription polymerase chain reaction method is applied to confirm theTNF-α,TNF-βin the degeneration of intervertebral disc tissue expression pattern.3) Group test statistical analysis results show that in the experimental group of theherniated disc tissue TNF, TNFR, sTNFRS is significantly higher than those in controlgroup of normal intervertebral disc tissue, suggesting that these substances involved in theintervertebral disc degeneration.4) At the same time, there is a correlation about the TNF, TNFR, sTNFRS (P<0.05),description biological interactions between inflammatory mediators and cytokines areinterrelated, influence the result of joint action.
