Objective To evaluate the sensitivity, specificity and clinical significance of coronary angiography(CAG) and myocardial perfusion SPECT in the diagnosis of coronary heart disease(CAD). Methods Three hundred and eighty-nine patients underwent both CAG (Siemens Coroskop Plus) and myocardial perfusion SPECT (GE Discovery VH). Two hundred and forty-eight cases were male(average age 54.26+11.51 years old), one hundred and forty-one cases were female(average age 58.07+9.63 years old). Results The clinical diagnosis showed 216 cases of CAD, 173 Cases of non-CAD in 389 patients. Among them, CAG showed 208 cases were positive, 171 cases were negative; myocardial perfusion SPECT showed 236 cases were positive, 153 cases were negative. The sensitivity and specificity of CAG in the diagnosis of CAD were 96.3% and 100% respectively; the sensitivity and specificity of SPECT were 86.1% and 71.1% respectively. The sensitivity of myocardial perfusion SPECT in the group of mild, moderate and severe coronary stenosis was 66.0%, 77.5% and 93.8% respectively. The sensitivity of myocardial perfusion SPECT in the group of single, two and three- vessel coronary artery stenosis was 77.9%, 89.7% and 94.5% respectively. Conclusion 1. The sensitivity of myocardial perfusion SPECT is related with the degree and the number of coronary artery stenosis. The sensitivity of myocardial perfusion SPECT is higher in the patients with severe and multi-vessel coronary artery stenosis, but lower in the patients with mild and Single-vessel coronary artery stenosis. 2. This study shows that the specificity ofmyocardial perfusion SPECT is lower, for 71.1%, so it has certain limitation in diagnosis of CAD. 3. The sensitivity and specificity of CAG is very high in the diagnosis of CAD. However, when CAG is normal myocardial perfusion SPECT is abnormal seriously, which is also important diagnosis index. So CAG should combine with myocardial perfusion SPECT in the diagnosis of coronary heart disease.
