Objective: To study the effect of hepatic artery ligation(HAL) on the liver by the model of choledochostomy with T tube drainage and HAL in dogs .Therefore,to provide experimental evidence for clinical surgery through observing the changes in the bile flow rate, liver function and hepatocyte ultrastructure after HAL.Methods: There are two parts in these experiments.In the first part, we gave 6 normal dogs the operation of choledochostomy with T tube drainage and jejunum fistulation.Then the liver function was observed at d-6, d0,d1, d7; bile was collected and measured every hour and transported to the jejunum through the tube of jejunum fistulation with the temperature of 38.5℃ (which is the bile temperature in the body of dogs). Took some liver tissue for the qualitative observation by electron microscope in 2 of the 6 dogs in random.Had the contrast examination of T tube to confirm the T tube unobstructed at the seventh day.In the second part,we gave the 6 dogs above the operation of HAL. Then the liver function was observed at 2h,4h,6h,12h and every day in the following two weeks; bile was collected and measured every hour and transported to the jejunum through the tube of jejunum fistulation with the temperature of 38.5℃(which is the bile temperature in the body of dogs). Took some liver tissue for the qualitative observation by electron microscope in 2 of the 6 dogs in random before HAL and 15 days after HAL.Had the contrast examination of T tube to confirm the T tube unobstructed at the fifth day.Results:The results of the first part 1. general conditions of the dogsPoor appetite and mind and little activities for two days after operation. Ameliorated from the third day and recovered in the fourth day. Body temperature was normal on the whole: 38.5C-39.5C.2. bile flow ratebile flow rate of normal dogs: 126.6 + 10.9ml/d or 5.59+0.07ml/kg.d;color and quality nature normal.3. liver functionThe change of aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin (TB), conjugated bilirubin (CB) and unconjugated bilirubin (UCB) between the preoperative and postoperative was not notable(compared with preoperative, ALT, AST, TB, CB, UCB of the first day and seventh day of postoperative were not notable different, P>0.05).4. results of electron microscopeHepatocyte form normal. The biggest reduction range occurred 2hs and it about 54% of the same time of preoperative; Bile flow rate decreased to the minimum in the fourth day: 54.6+5.5ml/d (about43.3% of the preoperative) ;65.2+5.9ml/d in the fourth day (about 51.1% of the preoperative) ; 65.2+5.9ml/d in the fourteenth day (about51.7% of the preoperative).From the fifth day,it showed a tendency of low level stably and had no significant difference compared with the fourteenth day(P>0.05). 3. liver function1) ALT, AST : They were both increased significantly after HAL(compared with preoperative,they were notably different in the ten days following HAL , P<0.01). The most significantly increase occurred within 12hs after HAL, especially within 2hs(ALT and AST increased to 30.667+4.8028U/L and 28.000+3.5777U/L ,about 1.87 times and 1.51 tunes of the preoperative separately); they both increased to the maximum in the fourth day: ALT 65.833+16.7262U/L(about 4.01 times of the preoperative);AST 56.500+12.6452U/L(about 3.04 times of the preoperative).
