Part I. The evaluation of bone metabolism indexs in patients with liver cirrhosis Objective: To determine the bone mineral denisity (BMD) and osteocalcin (BGP) parameters of patients with hepatic cirrhosis, investigate the relationship between cirrhosis of liver and bonal metabolism. So as to prevent patients with cirrhosis of liver from hepatic bone disease, improve their life quality .Methods: Fourty patients, 30 males and 10 females, and age-and-gender-matched controls were included. Bone mineral density was measured by using dual energy X-ray absorptiometry(DXA),The mean BMD value of the second, third and fourth lumbar vertebrae, as well as the BMD of the femoral neck troch and ward of proximal femur were used in the present analysis. A Z-score , which is the number of standard deviations from the mean BMD of a sex-specific, age-matched and weight-adjusted for the DXA-equipment, was evaluated by the use of a spine phantom and expressed as the coefficient of variation. Serum BGP was measured by using a radioimmunoassay. Result: 1. Compare indexes in patients with cirrhosis of liver to those in controls. Serum BGP levels in patients were significantly lower than that of healthy controls(3.34ng/ml±1.2 ng/ml vs 4.85ng/ml±1.24ng/ml,P<0.01); Bone mineral density indexes in patients with liver cirrhosis were lower than those of the healthy controls, but the difference was no significance (P>0.05). 2. According to the function of liver, divided the male cirrhosis patients into Child-pugh classification. Compare their relationship: Serum BGP levels in males cirrhosis patients were dropping as its Child-pugh grade was developing. There was significantly different between Child-pughA andC(P<0.05)and between Child-pughC and B(P<0.05), but the difference wasn't significant betweenB and A(P>.05); There was no signifcantly difference of bone mineral density among A,B,C three grades in male cirrhosis patients(P>0.05, respectively). 3. There were significantly positively correlations between serum BGP and bone mineral density{r=0.4329(neck), 0.4715(ward), 0.3586(troch),0.4866(L2-4) ,p<0.05 , respectively} Conclusions: 1. Serum BGP levels in patients with post-hepatitis liver cirrhosis were significantly higher than that of normal control, and there was significantly different between C and B and between A and C of Child-pugh classification in male cirrhosis patients . 2. Bone mineral denisity indexes in cirrhosis of liver were lower than those of controls, but the difference was no significance (P>.05). 3. There was significantly positively correlation between BGP levels and bone mineral density.Part2. Study on relationship between serum leptin levels and bonal metabolism indexes in cirrhotic patients Objective: Through investigate serum leptin levels relations with body mass index (BMI), gender, osteocalcin(BGP) and bone mineral density (BMD). understand the phthophysiology of malnutrition and osteodystrophy in patients with cirrhosis of liver. Methods: Thirty males and ten females patients with post-hepatitis cirrhosis and eight males and seven females respective control subjects were included in this study. All blood samples were drawn after an overnight fast and serum leptin levels were measured by using a radioimmunoassay. According to the liveral function, we divided the patients into Child-pugh classification.BMI was calculated by dividing weight (kg) by squared height(m2). The mean BMD value of lumbar vertebrae(L2-4)was included in present analysis. Result: 1. Serum leptin levels in females henlthy control were signifantly higher than that of males (8.58ng/±2.80ng/ml vs 3.73 ±2.80ng/ml, P<0.01); Serum leptin levels in females cirrhotic patients were higher than that of males (10.45ng/ml±2.82ng/ml vs 8.75ng/ml ±2.71ng/ml ), but the difference wasn'tsignifant(P>0.05). 2.
