| Background: Diseases of veins of lower extremity are common surgical illness. Primary deep venous valva insufficiency occupy about 53-69% of all, but the causes of the disease have not been clarified. Etiological factors accepted by most scholars are two theories, namely the venous valve theory and dissepiment theory. In the venous valve theory, venous valves will lose the function of preventing bleed from reflux because of the deformation, deletion or incompetence caused by high venous pressure. In the dissepiment theory, because the intensity of venous dissepiment decreases, the venous lumen will dilate which will cause the relative valve incompetence and high blood pressure in the vein, the vein dilataltion is the main morphological characteristic. But the quantitive clinical marker widespreadly identified for the vein dilataltion has not been discovered ,and study of correlation of the degree of the deep vein dilataltion on limb and the degree of severity of clinical symptom is absent.Object: This study was undertaken to investigate the relationships between lower limb symptoms and the diameter of the intermediate segment of femur and every segment of deep vein and the ratio of the diamter of the deep vein and artery in the same segment among normal and morbidit crowd are compared. Method: Twenty normal subjects and sixty patients with primary deep venous insufficiency were studied with ascending phlebography of deep vein on limbs and somographic estimates.All the subjects were divided into four groups according to standard of CEAP, and X-ray light sheet of every segment of deep vein was input to computers to splice using the software for processing image. After measuring the length of femur and selecting four segments according to the proportionalit, the ratio of the diameter of intermediate femur segment and superficial femoral vein and popliteal vein at every level was measured and the diameter of deep vein and artery in the same level. Result: Two methods manifest the ratio of the diameter of intermediate segment of femur and every segment of deep vein and the diameter of deep vein and artery in the same level, among normal and morbidit crowd have marked statistical difference (p<0.05). The ratio of the diameter of the intermediate segment of femur and every segment of deep vein and the ratio of the diameter of every segment of deep vein and artery have no marked statistical difference (p>0.05) in every morbidit crowd . The ratio of the diameter of intermediate segment femur and popliteal vein and the ratio of popliteal vein and popliteal artery have marked difference between mild group and moderate group(p<0.05),and have marked difference between mild and severe group, and have no statistical difference between moderate and severe group(p>0.05).Conclusion: The dilataltion of deep venous lumen is the main morphological characteristic; The nutrient alteration of skin in lower leg and the presence of ulceration have no significant correlation with reflux of superficial femoral vein and have significant correlation with reflux of popliteal vein. Reconstruction of the function of popliteal venous valve may have important role in the improvement of quality life and relief of clinical symptoms . The ratio of deep vein and ratio in the same level onlimb with color ultrasonography estimates have better objectivity . |
