| Objective: To investigate the genetic association with tumor necrois factor alpha (TNF-a) and interleukin-10 (IL-10) promoter single nucleoti-de polymorphisms (SNP) in the patients of Han nationality of Southern Jiangsu District with severe hepatitis B, and the possible associations with clinical manifestations of the disease.Methods: To this purpose, we compared 60 patients with severe hepatitis B with 57 chronic hepatitis B patients and 60 healthy controls. The TaqMan-MGB fluorescent probe PCR technique was used to examine the single nucleotide polymorphisms of TNF-a-308 and IL-10-592. The difference of distribution of genotypes, alleles of TNF-a- 308 and IL-10-592 were compared between the three groups. The clinical data were collected and relational analysis was performed. Serum concentrations of IL-10 were measured by ELISA.Results: There were no significant genotype distribution and allele frequencies of TNF-a-308 between either two of the subject groups. (P>0.05) In patients, no statistically significant association was found between genotype distribution of TNF-a-308 and clinical characters. (P>0.05) By contrast, the IL-10-592A allele was significantly higher in severe hepatitis B patients (79.17%) coripared with chronic hepatitis Bpatients (OR=1.218, x2=6.217, p= 0.045 <0.05), and healthy controls (OR=1.347, x 2=11.415,p=0.001 <0.01). Tliere was significant difference in the IL-10-592AA genotype distribution between those of the patients with severe hepatitis B and healthy controls, ( x 2=6.217,p=0.045 <0.05), or between those of the patients with severe hepatitis B and with chronic hepatitis B, ( x 2=11.084, p=0.004<0.01). Furthermore, in the patients with severe hepatitis B, statistically significant association was found between the IL-10-592 AA genotype and clinical characters such as prothrombin activity (PTA) and white blood cell (\/BC) counts. There was no significant difference in concentrations of IL-10 between either two of three genotypes of IL-10-592. (P>0.05)Conclusion: these data suggest that the IL-10-592A allele and the genotype of IL-10-592AA is one of the important factors that probably influence the pathogenesis, clinical course and prognosis of the patients with severe hepatitis B. Further study was needed since we did no find the relationship between the SNP of TNF-a-308 and severe hepatitis B in this present study. |
