Objective: To investigate changes of endothelium-dependent relaxing function in hypertensive patients with and without type 2 diabetes mellitus (EH+DM Vs EH ) and the effect of valsartan. Methods: With high-resolution ultrasound, we measured the change of diameter in brachial arteries in responding to reactive hyperemia ( AD%)which be used to reflect the whole endothelium-dependent relaxing function in 34 patients with essential hypertension and 28 patients with essential hypertension and type 2 diabetes mellitus pre and post treatment of valsartan, then use t test to compare their statistical difference. Results: No matter hypertensive patients with or without diabetes mellitus, their difference of fundamental inner diameter of brachial arteries (D0) pretherapy are not statistically significant, and change of diameter in brachial arteries in responding to reactive hyperemia (AD%) of patients with essential hypertension and type 2 diabetes mellitus was much reduced compared withessential hypertension group [(2. 50±4. 00)% VS (6. 34±4. 34)%, P<0. 05 ], while after being treated by valsartan for 12 weeks, their change of diameter in brachial arteries in responding to reactive hyperemia (AD%)were all ameliorated than pretherapy [ (4. 68 ± 3. 22)% VS (2. 50 ± 4. 00)%, (10. 12 ± 4. 68)% VS (6. 34 ± 4. 34)%, P<0. 05 ], but that of hypertensive patients with diabetes mellitus still much lower than those without diabetes mellitus (P <0.05). Conclusion: Hypertensive patients with type 2 diabetes mellitus suffered more serious functional impairment of endothelium-dependent vasodilation than those without type 2 diabetes mellitus, and this dysfunction may be ameliorated by treatment of valsartan for 12 weeks.
