Objective To evaluate MRI effectiveness of the diagnosis of pituitary microadenomas .Methods 56 patients with pituitary microadenoma were performed plain and GD-DTPA studied by means of GD-DTPA enhanced MR imaging. Plain scans were done by T1WI and T2WI FSE sequence at coronal and sagital plane. All cases were studied by dynamic MRI and were also scanned with conventional FSE sequence and FFE sequence after dynamic scanning. 50 minutes after injecting GD-DTPA , delayed enhanced MRI was performed. Results(l)FSE showed 48 cases(85.7%), FFE showed 51 cases(91%), FFE was superior to FSE(P<0.05). (2)Dynamic enhancement showed 55 cases(98.2%), delayed conventional contrast enhancement showed 48 cases(85.7%),delayed enhancement showed 33 cases(58.9%), The lesions were identified only on the dynamic sequence in 5 patients. Dynamic scan was superior to delayed conventional contrast enhancement and delayed enhancement (p<0.005). (3) Sensitivity and specificity of Dynamic were 98. 1% and 90.9%. Sensitivity and specificity of Dynamic combined with delayed conventional contrast enhancement(FFE) were 100% and 90.9%. Conclusion Half-dose Dynamic and delayed conventional contrast enhancement (FFE) improved sensitivity and specificity of lesion detection over plain MRI.Delayed enhanced MRI waste time and strength,rate of lesions detection was inferior to the dynamic and delayed conventional contrast enhancement.It should not be used as routine.
