Hypertension is a risk of cardiovascular events. Usually we use SBP and (or) DBP as indexes of measure, evaluation, treatment and forecast of cardiovascular events, so the dominated pattern of hypertension is control of SBP and DBP .While since 20 years ,lots of clinical investigations which got from different crowds provedthat the increase in pulse pressure was a independent risk of cardiovascular events, the forecasted valuation to cardiovascular events precede to SBP and DBP. Therefore , the dominated pattern of hypertension was changing gradually. Left ventricular hypertrophy is an ordinary target organ damage in hypertensives. Investigations proved that the burthen of left ventricular was increased in hypertensives , the reason of ventricular hypertrophy was the flexibility of big artery was decreased. It is important to evaluate the relationship between pulse pressure and the ventricular hypertrophy in essential hypertensive patients for preventing the situation and decreasing the risk .We monitored the clinic pulse pressure and applied echocardiography in 126 essential hypertensive patients who came to the third medical college of jilin university during 2002-2004, so we can discuss the relation of pulse pressure and left ventricular hypertrophy ,study whether clinic pulse pressure can reflect the degree of left ventricular hypertrophy. The 126 patients answered for the hypertension criterion of WHO/ISH, and the mean age was 57.7±9.5 years. They were excluded from non-essential hypertension, valvular heart disease, diabetes, hyperthyroidism, entourage vein disease, liver and kidney function obstruction according to illness history, body check, laboratory and cardiogram. We measured all patients height and weight, checked their blood sugar and blood fat., measured their blood pressure three times continuously and calculated the average number ,the number was the clinic blood pressure which we wanted to receive. We measured the AOD, LVIDD, IVSD, LPWD according to echocardiography. Body surface area (BSA,m2)=0.006H(cm)+0.013W(kg)-0.015, left ventricular weight(LVM, g)=0.8[1.04(IVSD+LVIDD+LPWD)3-LVIDD3]+0.6, left ventricular weight index(LVMI, g/m2)=LVM/BSA, we can assess LVH according to LVMI, left ventricular hypertrophy is fleshy when LVMI>20g/m2(men), LVMI>15g/m2(women). Using the left ventricular mass index (LVMI) as an index in evaluating LVH, the patients were divided into 2 groups: 54 cases in the LVH group and 72 in the non-LVH group, the age, sex, weight, the procedure of hypertension between two groups was no notability difference. Using the clinic pulse pressure as an index, the patients were divided into 3 groups: A group: pulse pressure < 60mmHg, 41cases; B group: 60mmHg≤pulse pressure<70mmHg, 44 cases; C group: pulse pressure≥70mmHg, 41cases, the age, sex, weight, procedure of hypertension among three groups was no notability difference. All data was delivered through means value ±standard errand( ±s). We compared the groups through t test or chi-square test, analyzed two variables with beeline correlation assay, it has significant difference when P<0.05.In this investigation, we found that SBP and pulse pressure in hypertensive patients who had LVH more increased significantly than those who had no, the difference was significantly (P<0.05). While the mean artery pressure and DBP had no difference between two groups(P>0.05).This clewed that pulse pressure and SBP were more important in forecasting LVH. Lots of tests proved that pulse pressure was a parameter which reflected the changing of big artery dilatation, therefore the main reason of cardiac muscle flashy may be descending of big artery dilatation which came from ascending of SBP and pulse pressure. The datum also proved that there was significant difference in diameter of main artery root between two groups, the diameter of main artery root was aggrandizement in LVH group.
