Objective:To investigate the relationship of pulse pressure and pulse pressure index to left ventricular hypertrophy in essential hypertensive patients.Methods:The protocols of 156 essential hypertensive patients in our hospital during last 3 years and 49 healthy people in our hospital physical check-up center were reviewed retrospectively. The essential hypertensive patients included 66 cases of male and 90 cases of female, age from 30 to 89 year, average age 63.7±12.2 years. The healthy people include 36 cases of male and 13 cases of female, age from 39 to 83 year, average age 60.2±12.0 years. The difference of BMI,age,SBP,DBP,PP,PPI,LVMI,HDL,LDL,TC,TG,EF,FS,the incidence of left ventricular hypertrophy were compared between the essential hypertensive patients group and the healthy people group. The 156 essential hypertensive patients were divided into subgroups: according to the PP(<60mmHg and≥60mmHg);PPI(<0.5 and ≥0.5);left ventricular hypertrophy(male LVMI>125g/m~2, female LVMI>120 g/m~2). Supine blood pressure was measured in the right arm with sphygmomanometer. LVMI,EF and FS were measured and calculated by transthoracic echocardiography. Age,BMI,the course of diseases,SBP,DBP,HDL,LDL,TC,TG,PP,PPI,LVMI,EF,FS,the incidence of left ventricular hypertrophy and the prevalence of left diastolic function decrease(A/E>1) were compared between subgroups. The correlation between PP and PPI was analyzed with Pearson correlation analysis. The relationship between LVMI and age,the course of diseases,SBP,PP,PPI was analyzed with multiple linear correlation analysis and multiple linear stepwise regression analysis.Results:1,There were no significant difference in age,BMI,HDL,LDL,TG,EF and FS between the essential hypertensive patients group and the health people group(P>0.05),but there were significant difference in SBP,DBP,PP,PPI,LVMI,TC and the incidence of left ventricular hypertrophy between the two groups(P<0.05).2,There were no significant difference in BMI,the course of diseases,DBP,HDL,LDL,TG,TC,EF,FS and the prevalence of left ventricular diastolic function decrease between the high PP subgroup and the low PP subgroup(P>0.05),but there were significant difference in age,SBP,LVMI and the incidence of left ventricular hypertrophy between the two subgroups(P<0.05). There were no significant difference in BMI,HDL,LDL,TG,TC,EF,FS and the prevalence of left ventricular diastolic function decrease between the high PPI subgroup and the low PPI subgroup(P>0.05), but there were significant difference in age,the course of diseases,SBP,DBP,LVMI and the incidence of left ventricular hypertrophy between the two subgroup(sP<0.05). There were no significant difference in BMI,HDL,LDL,TG between the left ventricular hypertrophy subgroup and the left ventricular nonhypertrophy subgroup(P>0.05),but there were significant difference in age,the course of diseases,SBP,DBP,PP,PPI,TC,EF,FS and the incidence of left ventricular diastolic function decrease between the two subgroups(P<0.05).3,The multilpe linear correlation analysis indicated that the age,course of disease,SBP,PP and PPI were positively correlated to the LVMI(P<0.05),and the correlation index is PPI > PP > age > SBP > the course of diseases. DBP had no obvious correlation with LVMI. There was significantly positive relationship between PP and PPI(P<0.01).4,The multiple linear stepwise regression analysis indicated that PP played the most important role and age played the second role in the development of left ventricular hypertrophy(P<0.01). The regression equation was as following: LVMI=44.935+0.533Age+0.560PP.Conclusions:The hypertension is a risk factor causing the left ventricular hypertrophy. The left ventricular hypertrophy in essential hypertensive patients is significantly correlated to the age,the course of diseases,SBP,PP and PPI, among which PPI plays the most important role and PP takes the second. PP has significantly positive relationship with PPI. PP plays the most important role and age plays the second role in the development of left ventricular hypertrophy.
