Background: In the later 1990s, many people were infected HIV for tansfusion in the middle rural area in our country,and they often co-infected other virus that can transmit via blood,such as HCV,HGV.Childbearing aged women who infected such these viruses can transmit them to their children by vertical transmission.Objective: To investigate of the transmission rate of HIV from mother to children. To analyze some factors that affect HIV vertical transmission. To see the traits of HIV children and to find a treatment regimen for AIDS children.Methods: The children aged 0-14 years old and their parents, and all women of childbearing age (20-45 years old) who ever were blood donation or transfusion were selected from a highly epidemic area of AIDS were interviewed and blood samples were taken for anti-HrV,anti-HCV,anti-HGV,anti-TTV and HBV-mark tests from January 2001 to April 2004.HIV-DNA or HIV-RNA were purified by PCR .The part of patient were tested for CD4+T cell count and HIV viral load.Results: Among 137 children who were born HIV positive mother,50children were positive for anti-HIV,the HIV vertical transmission rate was 36.5%(50/137).Among 107 HIV positive mothers ,there were 13 mother infected HIV by transfusion,and 8 women were transfused after post partum. 8 children who were born from HIV positive mother by post partum transfusion were all breast-feed, 3 children were positive,the MTCT rate was 37.5%(3/8) by breasting. Thirtyeight DNA fragment of HIV-1 gag gene PI7 were amplified by PCR from uncultured blood mononuclear cells (PBMCs) obtained from HIV-1 sero-positive High-risk women of children bearing age. Sequence analyses showed that all of them were HIV-1 B' subtype. The HIV vertical transmission rate among mother with AIDS (66.0%, 31/47) was significantly greater than that among HIV-carrier (21.1%, 19/90), P<0.05. Among 101 children aged 0-14years who their HTV positived mothers were co-infected HCV, 30 children were anti-HIV positive, Among 3 children aged 0-12years who their HIV positive mothers were HCV negtive, 1 children were anti-HIV positive .the HIV vertical transmission rates were29.7%(30/101) and 33.3%(l/3) respectively. The HIV vertical transmission rate among HIV positived mother who were co-infected HGV(22.4%,11/52) was significantly lower than that were no co-infected HGV(44.4%, 12/27). The HIV vertical transmission rate among HIV positived mother who were co-infected TTV(31.7%, 19/60) was lower than that were no co-infected TTV(50.0%,6/12).Among 60 children infected HIV, 47 cases were due to mother-to- child transmission (83.3%, 50/60), 8 cases were infected by blood transmission (13.3%, 8/60), two was due to other method (3.3%,2/60). 22 children infected HIV developed AIDS, among 15 AIDS children (average age were 4.5 years old) infected by HIV by mother-to-child transmission,7 cases were died. The primarily clinical manifestations of child AIDS were failure to thrive, chronic or recurrent fever, chronic or recurrent diarrhea, recurrent airway infection. 7 AIDS children are treating by HARRT. Their clinical manifestations were improved.Conclusion: The transmission rate of HIV from mother-to-child was 36.5% in our investigation, which was higher than that controlled by intervention (2%-6%). The transmission of HIV may were influenced by the status of mother's HTV or other virus infection .It's a crucial measure that prevent women of childbearing age infection HIV. HIV mother-to-child transmission was main method for child AIDS .The main reason led to child HIV/AIDS were the lack of the prenatal HIV counseling and testing for the high-risk women of childbearing age, unavailable interventions and the custom. Children HIV/AIDS had become a realistic problem when we prevented and cured AIDS in our country. HARRT is one of effective measures that prolong the life of children AIDS.
