Objective: Serum paraoxonase activity was measured by spectrophotometerboth in normal controls and cases with type 2 DM including cases withoutalbuminuria and with microalbumiuria and with macroalbumiuria based on theurinary albumin excretion rate (UAER),so did the levels of type IV collagen detectedby ELISA in groups. The levels of systolic blood pressure(SBP),diastolic bloodpressure(DBP),body mass index(BMI),fasting blood glucose(FBG),post bloodglucose(PBG) , hemoglobinA1c(HbA1c) , total cholesterol(TC) , totaltriglyceride(TG),high-density lipoprotein(HDL),low-density lipoprotein (LDL),blood ureanitrogen(BUN) , blood creatinine (Cr) et al were also measured.Furthermore, compared with those parameters in above groups .we also investigatedthe relationship among PON1 activity ,CIV and the possible correlation betweenevery one of them and UAER,BUN,Cr ,plasma lipid to determine the status ofthose variations in diabetic nephropathy(DN) and their effects on the onset anddevelopment of DN in order to study the significance of them in the pathogenesis ofDN. So that we may be offer some new and useful method to prevent and monitorDN. Methods: We use the case-control study .71 patients with type 2 DM (1999WHO diagnostic standards),38 male and 33 female, (58.6±11.1)years old. None hadpresent acute or chronic inflammatory disease history of liver or kidney diseasepulmonary fibrosis or connective tissue disease .Taken 18 persons as normal 4controls,10 male and 8 female, (56.39±5.59)years old. DM patients were dividedinto three groups based on UAER measurements, including 29 cases withoutalbuminuria (A group), 28 cases with microalbumiuria(B group) and 14 cases withmacroalbumiuria(C group). After absolute diet for 10 hours venous blood sampleswere obtained in the next morning. We measured the levels of serum PON1 activity(phenyl acetate) by spectrophotometer and the CIV by ELISA. The levels of SBP,DBP, BMI, FBG, PBG, BUN ,Cr,plasma lipids et al were also measured,meanwhile, the urine samples of 24 hours were taken for measuring albuminuria tocalculate UAER. We compared those data in above groups, furthermore, toinvestigate the relationship among PON1 activity ,CIV and the possible correlationbetween every one of them and UAER ,BUN, CR et al ,we also took the age ,DM duration,BMI,SBP,DBP,FBG,PBG,HbA1c as independent variablesto study the relationship or correlation among them. Continuous variables withnormal distributions are expressed as mean (S.D.); Continuous variables with skeweddistributions are expressed as median(inter-quartile range).The comparisons betweengroups were done using Student's t-test for independent samples or one-wayanalysis variance(ANOVA), correlation and multiple linear regression analysis wereapplied when appropriate. Logistic regression was used in order to evaluate thedanger of DN. If the variable was not normally distributed, then it wasloge-transformed prior to test. P value of <0.05 was considered statistical significant.All of above used SAS software. Results: The PON1 activity decreased significantly in DM groups than innormal control (NC) group(P<0.001),while the levels of CIV increased remarkablybetween above groups(P<0.001). Serum PON1 activity in the group with nephropathywas significantly lower than in the group with no complications(P<0.05), conversely,the levels of CIV increased significantly(also P<0.05).When we divided the DM 5patients into three groups by UAER, we found serum PON1 activity inmacroalbuminuria groups was significantly lower than those in microalbuminuria,none-albuminuria and NC groups(P<0.001),Serum PON1 activity was also significantly lower in microalbuminuria than those innone-albuminuria and NC groups(P<0.001).
