Objective: To establish and study the 2,4-Dichlorophenol (2,4-DCP) quantificational method for the measurement of the hematuria concentration. And the method could be primarily used in clinic to provide important information about some diseases.Methods: According to the heme in hemoglobin which had the similarperoxidase activity and could catalyze 2,4-DCP as the reactivereagent to form a red quinone dye which could be detectedspectrophotometrically. The direct method and depositing method were established,through groping for optimum reactive conditions and comparing with other methods,every analysis error of which was accepted.Considering the clinic convenience,the range of reference value was confirmed(mg/g·Cr) by the direct method and the application value was investigated in diseases through mensuring the urines of 500 healthy persons and the hematurias of 271 patients. Results: 1,Through a lot of experiments,the reactive reagents, surfactant and reactive medium were selected. The concentrations and the ratio of reagents,reactive time and reactive temperature were separately determined. And the automated chemistry analyzer method parameters were decided.2,Regression analysis of hematuria concentration measured by this method : y=0.0045x+0.3275, r=0.966. The lineal range of hematuria concentration was 0~400mg/L. The within-run CV and between-run CV and the mean recovery rate of the direct method and depositing method were separately measured by different concentrations of hematuria specimens. 3,There was no interference of bilirubin in urine which was below 240 μmol/L(P>0.05).The concentration of vitamine C below 350μmol/L was no interference to the results(P>0.05).But if the vitamine C concentration was up to 550 μmol/L,the measured absorbance was only 70 percent of the former absorbance. 4,The urinary Hb concentrations of 500 healthy persons presented normal distribution by the direct method through SAS 6.12 software analyzing.The urinary Hb difference of each age group was no statistical significance (P>0.05).But there was a significant difference (t=3.12,P<0.01) of which between sex groups.The normal urinary Hb concentration of man was 0~17.6 mg/g·Cr,and the woman's was 0~23.8 mg/ g·Cr . 5,The direct method was used to investigate the presence of hematuria in 171 patients of non-neoplasm urologic diseases,and the trends which of urinary Hb,serum creatine and carbamide of 20 renal transplantation patients who exhibited acute rejection. The method could provide important information for these.Conclusions: The hematuria was one of ordinary and important symptoms in clinic of urologic diseases. 2,4-DCP quantificational method was established because there was moreinterference to the determination of urinary Hb concentration andthe methods for measuring hematuaia mostly were qualitativeand semi-quantitative. It not only had the better sensitivity,butalso could quantificationally measure the hematuriaconcentration through the automated chemistry analyzer and themanual assay , which could be applicable to hospital of different conditions.The method could exert important effects ondetermining the hematuria of some diseases patients.
