Leptin is a hormone that regulates metabolic efficiency,energy expenditure.It is mainly produced in adipose.Now we acknowlege that leptin is also present in placenta.It can mediate the excretory function of cytotrophoblast and vascular growth. Leptin participates in pregnancy complication.TNF-α is a short peptide that mainly produced by activated macrophages,it is important immunol regulatory factor and has close association with pregnancy.When abnomal increse in plasma,it may impair the vascular endothelium ,then may result in abnormal formation of endothelium cell and subsequnt increased microvascular filtration capacity.We have documented that leptin receptor is belong to cytokin family type I and has common sub–unit for one hundred and thirty gp,they contact with different receptor spot,so we conclude that they have some function association.The theme's aim is that examine the levels of leptin and tnf-αin controls and preeclampsias by immunohistochemitry in order to analyze the mechanism of preeclampsia further and explore new idear. ObjectiveTo investigate the mature placental expression of leptin and tnf-αin women with preeclampsia and study their relationship in order to study the mechanism of preeclamsia from immunlogy. Methods Immunohistochemitry was used to examine the levels of leptin and tnf-αin 25 controls and 66 preeclampsias.Preeclampsias were divided into mild,moderate and severe groups. Results 1.Immunolocalization of leptin/TNF-α was founde in syncytotrophoblast and decidual cells.2.Levels of leptin expressed in milde and severe groups were higher than that in controls (p<0.01,0.01) and no difference between controls and mild group (p>0.05).3. Levels of tnf-αexpressed in all preeclampsias were higher than that in controls (p<0.05,0.01,0.01). 4.There were significantly positive correlations between leptin and tnf-αin milde and severe preeclampsia groups(r=0.571,p<0.05及r=0.657,p<0.01). Conclusions1.Increased leptin in preeclampsia placenta may impair the ability of cytotrophoblast invasion of spiral arterioles and ichemia of placenta vascular.Placenta dysfuntion may appear.The activation of maternal vascular endothelium result in preeclampsia.2.The expression of TNF-αwas up graded in preeclampsia cases which minimise the intrusion ability of spiral atery,then impair invasiveness during implantation and placentation .Placenta ischemia as a rusult of recontruction of uterus atery lead to less oxygen which destroy the balance of metabolism.Further, preeclampsia was a result of widespread dysfuction of maternal vascular endothelium .3.Increased TNF-αpromoted the synthesis of leptin. Leptin was upgraded by tnf-α. Leptin and tnf-αmay contribute to the pathology of preeclampsia.
