PrefaceTo discuss the differences between 3D color power angiography (3D -CPA) and pathological microvascular density ( MVD) in the differentiation of benign and malignant ovarian tumor, and at the same time to evaluate the clinical efficacy about the 3D - CPA in differentiation of benign and malignant ovarian tumor.Materials and MethodsWe examined 43 women patient who were suspected acquiring ovarian tumor (post -operation pathological diagnosis: 21 malignant, 22 benign) using 3D - CPA imaging technique to classify the tumor vessels qualitatively, and then made the diagnosis of benign or malignant. The specified procedure was to define the spatial arrangement of tumor peripheral and internal into 3 grades; Grade I; tumor vessels were detected (or not) only in peripheral region, no internal vessels were detected; Grade II: tumor vessels were detected both in peripheral and internal region, but only a few were detected which manifested as spots or bar - like and with fewer branching; Grade III: tumor vessels could be detected both in peripheral and central, and most of which located in the central part and manifested as larger diameter, more branching and tortuous distribution. After the operation, we dyed the specimens with the antibody of vascularendothelial growth factor CD34 and by using immunohistological SP stain we dyed the pathological slices to count the MVD. The more specified process was at first in the low - resolution microscopic field, we defined an area in which with denser neo - vessels, then using the high - resolution to count the mean value of MVD of the 3 continuous fields as the MVD value of this specimen. After that, we made the statistical analysis about the results of the MVD, 3 D -CPA and pathological diagnosis to show if they were interrelated. We used grade - relating test to analyze the results of MVD and 3D - CPA to show if they acquired the congruity.ResultThe t - test data of MVD and pathological results indicate: great differences existed in the expression of MVD in benign and malignant ovarian tumor. The malignant tumor expressed apparently more than the benign ( P <0. 001) , MVD may as a criterion for making a differential diagnosis. The t - test data of 3D -CPA and pathological results indicate; 3D - CPA acquires a specificity of 100% , correctness of 81.4% , sensitivity of 63.7% , positive expectant value of 72.4% , negative expectant value of 100%. These illustrate the high specificity of 3 D - CPA in differentiating benign and malignant ovarian tumor, as it has an accordance of 81.4% with the pathological findings; however, it also has a high false - positive rate. The grade - relating test of the results of 3D - CPA and MVD indicates: in the differential diagnosis of benign and malignant ovarian tumor, 3D - CPA acquires the same efficacy as the MVD. No obvious differences could be observed between these two methods (Grade - relating index = 0.568, P <0.001). However, MVD evaluation could only be performed after the operation, then 3D- CPA could be used to indirectly evaluate the tumor vas-cularization and it is a effective differential diagnosing method in clinic.Discussion3D - CPA as a kind of non - invasive imaging examine method may use in...
