Purpose: To discuss the diagnostic value of transrectal ultrasound in diffuse invasiveprostate carcinoma. Methods: A total of 59 patients including 21 prostate carcinomas and 38 moderate orsevere benign prostate hyperplasias, diagnosed by histopathologic findings, weredetected by transrectal ultrasound. In the 21 prostate carcinomas 16 were classified asdiffuse invasive type. Prostatic modality, borderline, echo in the gland and colorDoppler imaging were generally observed. At the same time RI values weremeasured. Results: Irregular borderlines were found in 13 diffuse invasive prostate carcinomas(81.25%) and 1 benign prostate hyperplasia (2.63%). Unclear borderlines betweeninner and outer gland were found in 12 carcinomas (75%) and 11 hyperplasias (28.95%). And extremely inhomogenous echo was found in 13 carcinomas (81.25%) and 5hyperplasias (13.16%). No obvious significance was found in color Dopplerimagings and RI values between the two groups. Conclusions: Prostatic borderline, inner echo and borderline between inner and outergland are quite important in the differentiation of diffuse invasive prostate carcinomaand moderate or severe benign prostate hyperplasia. Color Doppler imaging and RIvalues are useless for the differentiation.
