Objective : To evaluate the distribution of HBV infection marker in Shandong Jiyang of 15~40 years old people;Observe adult's immunogenicity in different schedule to yeast-derived hepatitis B vaccine(YDV);Analyse HBV DNA in the only'anti-HBc'positive serum. Methods:In 2003 November,20 villages in Shandong province Jiyang was selected by cluster-random design and vein blood was gathered in 15~40 years old people. HBsAg,anti-HBs,anti-HBc and HBeAg,anti-HBe were measured by SPRIA and ELISA test kit. HBV susceptible of 15~40 years old are assigned to two groups, 0-1-6 months group and 0-6 months group.They were vaccined with 10μg/1.0ml of YDV made by Shenzhen kangtai Biological Products respectively. After 1 month of full vaccination schedule,the anti-HBs concentration is measured by SPRIA. HBV DNA was detected in only 'anti-HBc'positive serum by PCR method. HBV DNA was purify by heat-method. Amplification of HBV S gene was performed using primers specific for HBV surface regions, and the product of PCR was detected by electrophoresis. Results:The cross-sectional study shows that HBsAg prevalence rate, anti-HBs prevalence rate , anti-HBc prevalence rate and HBV total infection rate were 6.52%,24.77%,30.99% and 43.63% in Shandong Jiyang of 15~40 years old people. The HBsAg prevalence rate increased along with the age increasing(P<0.05). HBeAg positivity rate decreased along with the age increasing. Anti-HBe positivity in 15~group is lower than older-age group. The effective seroconversion rate of anti-HBs were 95.34%,86.41%,the mean geometric concentration(GMC) of anti-HBs were 332.5 mIU/ml(95%CI 280.6~394.0 mIU/ml)and 214.6mIU/ml(95%CI 176.8~260.5 mIU/ml) after 1 month of full vaccination schedule in 0-1-6 months group and 0-6 months group respectively. The effective seroconversion rates and GMC in 0-1-6 months group was significantly higher than group of 0-6 months(P<0.05). Logistic analye show that seroconversion rate is related with immune schedule and sex, but there is no relationship between age and seroconversion rate. GMC shows no difference in sex-group and age-group. There is 114 only 'anti-HBc'positive serum in the screen objects. 4 out of them are HBV DNA positive,the rateis 3.5%(95% CI 0.13%~6.89%). High Co/s value shows high HBV DNA positivity rate. Conclusions:HBsAg prevalence rate of 15~40 year old people were 6.52% in Jiyang,which is lower than the whole country investigation. But the rate is similar to the Shandong province's HBsAg prevalence rate 7.49%. HBeAg positivity rate decreased along with the age increasing. 10μg/ml YDV was immunogenic to vaccinate 15~40 years adults. The effective seroconversion rates and GMC in 0-1-6 months group was significantly higher than group of 0-6 months. 0-1-6 months schedule of 10μg/1.0ml YDV is recommended. A proportion of only 'anti-HBc'positive cases are HBV DNA positivity. High anti-HBc titre is associated with high HBV DNA positivite rate, which sustaines that HBV DNA positivite rate have positive relationship to the anti-HBc titres.
