Objective To evaluate the effects of ulinastatin(UTI) on Ischemic-Reperfusion Injury during piggyback orthotopic liver transplantation (PBOLT) operation ,the comparing study between UTI-used group and the control one on acute inflammatory action factor,oxyode free radical,liver function,kidney function and lung function have been launched.Methods twenty patients with end-stadge liver diseases undergoing PBOLT were randomly divided into two groups:ulinastatin group received intravenous infusion UTI continuously during the whole operation(group U,n=10)and control group received the same amount of normal asline instead of UTI.Internal jugular vein blood samples were collected to measure plasma concentration of tumor necrosis factor alpha(TNF- α ), interleukin-1 (IL-1), interleukin-6(IL-6), malodialdehyde(MDA), superoxide dismutase(SOD),liver function(ALT,AST),serum beta2-microglobine( β2-GM),urine beta2-microglobine(U β2-GM) ,respiration index(RI)and oxygenation index(OI) on 6 time points:before skin incision(T1),120min after skin incision(T2),30min in anhepatic phase(T3),5min, 60min and 120min in reperfusion phase(T4,T5,T6). Results The levels of TNF- α , IL-1, IL-6, MDA, ALT,AST, (β2-GM ,U β2-GM and RI were significantly increased at T4 and kept high levels on the following times in group C(P<0.05);the changes were not significant in group U.Conclusion (1)Acute inflammatory actions caused by ischemia-reperfusion injury were observed during the PBOLT (2)The protective effects to liver function,kidney function and lung function of UTI exist on Acute inflammatory actions during PBOLT.
