| AimMND is a relatively uncommon disease with annual incidence of 0.4 - 1. 76 /100,000. To date, there is no effective treatment of MND. The present study is designed to investigate the clinical application problems of self - marrow stem cell transplantation in the treatment of MND by observing its treatment effect.Materials and Methods1. subjects;48 patients of MND rolled in the hospital from March 2004 to October 2004.2. methods: prior to the operation ,the following lab tests were performed : blood routine, urine routine, hepatic function, renal function, blood glucose, blooding time, coagulant time, ECG. First, marrow puncture was performed to get marrow with the volume of 100 - 160 ml in each patients. Then, marrow was sent to the lab to separate marrow cells, get the volume of 2. 5 -5 ml, which was intrathecally injected in patients to observe the treatment effect.3. statistical analysis: the data were expressed as mean ± SE. Changes in WBC , hepatic and renal function, and electrolyte were analyzed by pair -t test with the software of SPSS11. 0. P<0.05 was statistically significant.ResultsThe patients showed obvious decrease in muscle tension and muscle tremor , improvement of dysarthria, dysphagia , difficulty in lifting the head, and muscle strength, with different degrees. The following visit results showed that average improvement time last 9 weeks. Some patients showed fever, headache, nausea, vomit, and etc. But the mentioned symptoms above would be disappeared in 1 - 4 days. WBC numbers (9,2 ±1.3) following transplantation in patients were significantly high, compared with those prior to transplantation (7. 0 ± 1. 2) , but no significant difference in hepatic and renal function and electrolyte.DiscussionTo date, there is no effective treatment methods of MND, due to unclear underlying mechanism. In the present study, Self - marrow stem cell transplantation was observed to improve some symptoms of MND patients, which has been never reported. The reason is nuclear. We propose that it may be related to the following factors.Marrow stem cell may be differentiated to neuron in some condition, with resulting in the rebuilding of neural circuts.Transplanted marrow stem cells may secrete some substances, which enhance or inhibit the activities of some neural structure related to the regulation of muscle tension, resulting in the decrease in muscle tension.There are some motor neurons in CNS in patients. After the transplantation, the remained neurons may be stimulated to function, contributing to the symptoms improvements.The transplantation mixture may have effect on some neurotransmitters in CSF, which may contribute to the symptoms improvements. |
