PrefacePostpartum thyroiditis (PPT) is a thyroid disorder that presents during the first postpartum year with no history of thyroid diseases before delivery. It's a kind of autoimmune thyroid disease. Women can develop this disorder after normal delivery or abortion. Since Amino first proposed the conception of PPT in 1977,many scholars have investigated it. It's characterized by the development of transient hyperthyroidism and/or hypothyroidism during the first six months of the puerperal period . The uptake of I is low and the serum titer of thyroid per-oxidase antibody elevates as well as lymphocytic infiltration of the thyroid gland.There are reports of associations of HLA - II antigens with postpartum thyroiditis in Caucasians from Canada,Denmark,Sweden and the United States. Till now there is not any study about PPT and HLA - II in China. In this study ,we use the method of PCR/SBT( Sequence based typing) to investigate the polymorphism of HLA - DPA1,DPB1 ,DQA1,DQB1 with the susceptibility of this disorder in Shenyang Hans.Methods[Subjects] 488 parturients with term birth received follow - up for 6 months by our group since Sep,2002 in Shenyang. All of them were Hans. 35 parturients were diagnosed as clinical PPT( CPPT) and 23 parturients were diagnosed as subclinical PPT(SPPT).31 CPPT patients , 21 SPPT patients and 82 normal controls were involed in this study.[ Diagnostic standard] (1) No history of thyroid diseases before delivery; (2) Thyroid dysfunction developed during the first six months of the puerperal period . PPT was classified as overt and subclinical according to free thyroid hormones. (3)Negative TRAb.[Experiment methods] Extracted genomic DNA from anticoagulant whole blood and then amplified the exon 2 of HLA - DPA1,DPB1,DQA1,DQB1 using the method of PCR. The SBT method was used to type the genes. Patients and controls positive for an allele(phenotypic allele frequency) were compared by x~2 test. Pc = P x the numbers of alleles tested. Significance was defined as Pc <0. 05.Results1.1 The comparision of the allele frequency of HLA - DPA1 locus between PPT patients and controls(C) : 7alleles were detected. There was no significant difference between PPT patients and controls.1. 2 The comparision of the allele frequency of HLA — DPA1 locus between CPPT^SPPT and C:There was no significant difference between CPPT/C,SPPT/ C,CPPT/SPPT.2.1 The comparision of the allele frequency of HLA - DPB1 locus between PPT patients and controls: 21 alleles were detected. The frequency of DPB1 * 0301 was significantly increased in PPT,but the P value was not significant after correction.2.2 The comparision of the allele frequency of HLA - DPB1 locus between CPPT,SPPT and C: the frequency of DPB1 * 02012 was significantly decreased in SPPT compared with CPPT and C ,but the P value was not significant after correction; the frequency of DPB1 * 0301 was significantly increased in CPPT compared with C and the frequency of DPB1 * 0401 was significantly increased in CPPT compared with SPPT,but there was no significance after correction.3.1 The comparision of the allele frequency of HLA - DQA1 locus between PPT patients and controls: 8 alleles were detected.
