Objective: To investigate 1L-21 and autoimmune thyroid disease(AITD)(including Graves disease and Hashimoto's thyroid gland)by detecting serum IL-21 expression in patients with Graves disease(GD)and Hashimoto's thyroiditis(HT).The correlation between thyroid function and other biochemical indicators in patients with inflammation provides new ideas for the therapeutic effect and prognosis of AITD.METHODS: From November 2017 to March 2018,patients who were admitted to the Department of Endocrinology at the First Affiliated Hospital of Shihezi University School of Medicine had patients who met the inclusion criteria.The study group was 95 patients in the GD group(54 in the GD-untreated group,GD).-41 patients in the treatment group;38 males and 57 females,aged 56.78±11.58 years old,94 patients in the HT group(44 patients with HT hypothyroidism,50 patients with normal HT thyroid function;39 males and 55 females)The age was 55.19±12.25 years old,and the healthy control group was 76 cases(36 males and 40 females)with an age of 53.61±14.05 years old.Serum IL-21 levels and thyroid function and other biochemical indicators were measured in each group.All data were entered using Epidata 3.1 software to complete all data entry,and statistical analysis was performed using SPSS 20.0 software.Results were analyzed by analysis of variance or dunnett-t test and correlation analysis.Results:(1)Serum thyroid hormone(FT3,FT4,TT3,TT4)and autoantibody(TPOAb,TGAb)levels were significantly higher in the GD-untreated group than in the control group(P<0.05);GD-untreated group Serum thyroid hormone(FT3,FT4,TT3,TT4)levels were significantly higher than GD-treated group,HT-hypothyroidism group,HT-thyroid function group(P<0.05),serum thyroid hormone in GD-treated group.(FT3,FT4,TT3,TT4)were significantly higher than the HT-hypothyroidism group,the HT-normal group and the healthy control group(P<0.05).The levels of thyroid autoantibodies(TPOAb,TGAb)in the HT-normal group and HT-hypothyroidism group were significantly higher than those in the control group(P<0.05).(2)Serum IL-21 level(210.57±123.37)pg/m1 in GD group was significantly higher than that in control group(145.86±33.34)pg/m1(P<0.05),serum IL-21 level in HT group(214.67±82.93)pg /ml was significantly higher than the control group(145.86 ± 33.34)pg / m1(P <0.05).The serum IL-21 level(258.49±146.67)pg/ml in the GD-untreated group was significantly higher than that in the GD-treated group(147.46±9.02)pg/ml(P < 0.05).Serum IL-21 levels in HT-hypothyroidism group(228.89±96.25)pg/m1 and HT-methyl group(202.16±67.67)pg/m1 serum IL-21 levels were higher than the control group(145.86±33.34)pg /m1(P<0.05);serum IL-21(228.89±96.25)pg/m1 in HT-hypothyroidism group and serum IL-21(202.16±67.67)pg/m1 level in patients with normal HT-thyroid function The difference was not statistically significant(P>0.05);serum IL-21(228.89±96.25)pg/m1 in HT-hypothyroidism group and serum IL-21 level in HT-methyl group(202.16±67.67)pg/m1 It was higher than GD-treated group(147.46±9.02)pg/ml(P<0.05).(3)Serum IL-21 level in GD group was positively correlated with FT3,FT4,TPOAb and TGAb(r=0.109,r=0.137,r=0.348,r=0321;P<0.05),and negatively correlated with TSH(r=-0.034,P>0.05).There was no significant correlation between serum IL-21 and thyroid-related indicators in the HT group.(4)TC level(3.80±0.71)mmol/l in GD group was better than healthy control group(4.19±0.75)mmol/l and HT-methyl normal group(4.28±1.03)mmol/l and HT-hypothyroidism group(5.41±0.51)mmol/l decreased(P<0.05);GD group TG level(1.03±0.37)mmol/l and control group TG level(1.35±0.88)mmol/l compared with HT-hypothyroidism group(2.44±1.27))mmol/l decreased(P<0.05);HDL levels in GD group,HT-hypothyroidism group and HT-methyl group decreased compared with the control group,but there was no significant difference between the groups(P>0.05);GD The LDL level of the group(1.93±0.58)mmol/l was lower than that of the normal control group(2.35±0.46)mmol/l and the HT-methyl group(2.63±1.58)mmol/l,and the HT-hypothyroidism group(2.85±0.79).Methyl / l decreased(P <0.05),TC-hypothyroidism group TC(5.41 ± 0.51)mmol / l,TG(2.44 ± 1.27)mmol / l,LDL(2.85 ± 0.79)mmol / l level compared with healthy control group Increased(P<0.05).Serum CYS-C level(1.01±0.35)mg/L in GD group was higher than that in control group(0.82±0.22)mg/L,HT-hypothyroidism group(0.72±0.13)mg/L(P<0.05),HT-The level of CYS-C in the hypothyroidism group was lower than that in the GD group and the HT-methyl group(0.95±0.23)mg/L and the control group(P<0.05).Conclusions:(1)Serum IL-21 may be involved in the pathogenesis of Graves disease and Hashimoto's thyroid disease.(2)Serum IL-21 levels are associated with thyroid hormone levels in GD.
