A Study On Effect Of Psychological Intervention On Unhealthy Emotion And Living Quality Of Ankylosing Spondylitis Patients And Its Relative Factors

Objective: 1. To survey on both unhealthy emotions and living quality changes of AS inpatients before and after psychological intervention. 2. To probe into the influence of psychological intervention on the unhealthy emotion of them. 3. To probe into effect of psychological intervention on the living quality of AS patients. 4. To study primarily on influencing factors of living quality of AS patients. Method: a total of 82 AS patients were performed one-month follow-up investigation by adopting case-control method, 42 of them were accepted combined psychological intervention, the other 40 cases were accepted treatment as routine. All cases were investigated on their common information and physiological indexes by filling out the short form–36 health survey questionnaires (SF-36), social support rating scale (SSRS), Eysenck personality questionnaire (EPQ), medical coping mode questionnaire (MCMQ), self-rating depression scale (SDS) and self-rating anxiety scale (SAS). Result: ⒈ There was no significant difference in terms of the physiologic indexes changes between before and after treatment in AS patients (t-test, P>0.05). ⒉ Through Repeated Measures analysis of variance (ANOVA) on continuous changes of both SDS and SAS total scores of all cases assessed three times during whole therapy showed that: ① with the treatment continuous conducted, the value of SDS score of patients of non-intervention group was decreased gradually (F=4.501, p=0.031), while there was remarkable descending of SDS scoring in patients of psychological intervention group (F=0.681, P=0.045). ② The scores of SAS in non-intervention group AS patients of tended to be decreased gradually with the treatment (F=0.681, P=0.047). And it showed a similar decreasing in psychological intervention group (F=0.383, P=0,602). ⒊Compare with non-intervention group,the four factors scores including physical functioning, bodily pain , vitality and role limitations due to emotional problems on intervention group were improved(P<0.05)⒋ The multi-factors gradual regressive analysis on influencing factors of SF-36 living quality showed that: EPQ-N has influenced on their physiological function (PF) in AS patients. The education level of AS patients has influences on their role physiological (RP). Both total SDS score and gender are influencing factors of body pain (BP). Total SDS score and objective support are related with their general health (GH) conditions. Vitality (VT) of patients influenced by both their total SDS score and average family income. Factors including total SDS score, total SAS scores, EPQ-E, education level, and subjective support of AS patients influenced their social functions. Role emotional (RE) of AS patients was influenced by their education level, avoidance, and EPQ-L. The mental health (MH) of patients influenced by surrender and their average family income. Conclusion: ⒈ Psychological intervention could palliate the depression of AS patients remarkably. ⒉ The living quality of patients with AS can be promoted significantly by psychological intervention. ⒊ The influencing factors of living quality in AS patients are including gender, education level, average family income, social support, coping modes (avoidance and surrender) and personality trait.
